摘要
目的探讨阿托伐他汀治疗脑梗死患者颈动脉粥样硬化(AS)斑块的效果。方法 124例脑梗死患者,采用抽签法将其分为实验组和对照组,每组62例。对照组针对患者病情采用常规治疗并给予烟酸等非他汀类降脂药治疗,实验组在常规治疗的基础上给予阿托伐他汀钙片治疗。比较两组治疗前后血脂及颈动脉粥样硬化斑块的变化。结果治疗后,实验组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、颈动脉粥样硬化斑块面积及斑块厚度[(4.55±1.40)mmol/L、(2.15±0.52)mmol/L、(2.78±0.61)mmol/L、(0.83±0.64)mm^2、(0.82±0.49)mm]较治疗前[(6.51±1.36)mmol/L、(2.42±0.89)mmol/L、(3.93±0.88)mmol/L、(1.35±0.52)mm^2、(1.35±0.56)mm]和对照组[(5.94±1.10)mmol/L、(2.39±0.72)mmol/L、(3.42±0.80)mmol/L、(1.33±0.44)mm^2、(1.29±0.51)mm]明显降低,高密度脂蛋白(HDL)[(1.48±0.32)mmol/L]较治疗前[(1.34±0.35)mmol/L]和对照组[(1.35±0.36)mmol/L]明显升高,差异均具有统计学意义(P<0.05)。结论颈动脉粥样硬化斑块的脑梗死患者采用阿托伐他汀治疗,可有效改善颈动脉粥样硬化的斑块状态,提高斑块稳定性,是脑梗死伴颈动脉粥样硬化斑块较好的治疗药物。
Objective To explore effect of atorvastatin in treating carotid atherosclerotic plaques of cerebral infarction patients. Methods A total of 124 cerebral infarction patients were divided by lottery method into experimental group and control group, with 62 cases in each group. The control group received conventional treatment and non-statins lipid-lowering drugs such as nicotinic acid by pathogenetic condition, and the experimental group received conventional treatment and atorvastatin calcium tablets for treatment. Changes and comparison were made on blood lipid and carotid atherosclerotic plaques. Results After treatment, the experimental group had lower total cholesterol(TC), triglyceride(TG), low density lipoprotein(LDL) and area and thickness of carotid atherosclerotic plaques[(4.55±1.40) mmol/L,(2.15±0.52) mmol/L,(2.78±0.61) mmol/L,(0.83±0.64) mm^2 and(0.82±0.49) mm]than [(6.51±1.36) mmol/L,(2.42±0.89) mmol/L,(3.93±0.88) mmol/L,(1.35±0.52) mm^2 and(1.35±0.56) mm] of before treatment and [(5.94±1.10) mmol/L,(2.39±0.72) mmol/L,(3.42±0.80) mmol/L,(1.33±0.44) mm^2 and(1.29±0.51) mm] in the control group,higher high-density lipoprotein(HDL)[(1.48±0.32) mmol/L] than[(1.34±0.35) mmol/L] of before treatment and [(1.35±0.36) mmol/L] in the control group, and differences had statistical significance(P〈0.05). Conclusion For carotid atherosclerotic plaques of cerebral infarction patients, atorvastatin can effectively improve state of carotid atherosclerotic plaques and increase stability of plaques. It is a perfect medicine for the treatment of cerebral infarction combined with carotid atherosclerotic plaques of cerebral infarction.
