摘要 目的探讨四肢滑膜肉瘤的磁共振(magnetic resonance imaging,MRI)表现,提高对其诊断水平。方法回顾性分析本院经手术病理证实的13例四肢滑膜肉瘤的临床及影像资料,所有患者均行平扫及增强扫描。结果 13例患者共13处病灶,分别为上臂5例,大腿5例,足部3例。8个病灶呈椭圆形,5个呈类圆形。病灶最大直径16cm,平均7.9cm;MRI上肿瘤信号不均,分隔、坏死、囊变、出血常见,多呈显著不均匀强化。结论 MRI能清楚显示滑膜肉瘤的部位、大小及其与周围组织的关系,有一定的特征性,但较小的滑膜肉瘤可显示相对均匀的信号和清楚的边缘,类似良性病变,仍需病理确诊。 Objective To inverstigate the MRI manifestations of synvial sarcoma in limbs and to improve its diagnostic accura- cy. Methods The imagining and clinical data of 13 cases of synvial sarcona proved by pathlolgy were analyzed retrospectively. All cases had been performed contrast-enhancement MRI, Results Thirteen were detected in 13 synvial sarcona, whith were lo- cated in the upper limb in 5 cases, in the thigh in 5 eases, in the heel in 3 cases. 8 cases lesions showed oval shape, and 7 le- sions showed round shape, The lesions ranged in maximum diameter from 3.5 to 16 cm, with a mean of 7.9 cm. Their MRI ap- pearance was usually heterogeneous, seta, hemorrhage and necrotic or cystic changes were frequently seen. The enhancement of synovial sarcoma was usually pronounced and inhomogeneous. Conclusion MRI can demonstrate the location, site and relation- ship with surrounding tissue of synovial sarcoma clearly. The characteristics of MRI findings are specific. But small synvial sarco- ma can demonstrate relatively homogeneous signal intensity with well circumscribed margin, still require biopsy to confirme.
