摘要
目的研究几丁糖电纺膜完全修复硬脑膜缺损后,其下方新生纤维膜与正常硬脑膜的差异。方法健康成年的新西兰家兔16只,取头部正中切口在双侧颅顶部对称开直径约1.2cm大小的圆形骨窗;剪去0.8mm×0.8mm大小的硬脑膜组织,并将合适大小的几丁糖膜置入实验侧硬膜缺损区;对照侧不进行硬脑膜切除作对照,缝合皮下及皮肤后进行饲养,术后8周(8周后硬膜已完全重建)处死,观察骨窗周围大体情况,通过组织学检验几丁糖膜下方新生纤维膜与正常硬膜差异。结果 1术后见皮肤及皮下无红肿、无渗出、化脓等炎症反应;几丁糖膜被严密包裹;从正中线切开包裹组织,见膜周围无粘连,容易被取出;膜下有新生纤维膜形成,严密封闭脑组织,无脑脊液漏;新生纤维膜与脑组织无粘连;2新生纤维膜组织与正常硬膜对比:HE染色提示实验组术后8周几丁糖膜下新生纤维膜为纤维结缔组织,Masson提示其中大量纤维素及胶原形成,免疫组化提示新生纤维膜已完全成熟,无b FGF表达,未见EGF表达,为完全成熟的纤维结缔组织,与正常硬脑膜无明显差异。结论新型几丁糖膜修补硬脑膜效果确切,且新生纤维膜与正常脑膜无明显差异。
Objective To study the difference of normal dura mater and newly grown dura mater after complete repair of dural defects with chitosan electrospun membrane. Methods Round bone windows (diameter=1.2cm) were operated on sixteen healthy adult New Zealand rabbits on bilateral sides symmetrically near the medial line. The dura mater was removed 0.8 mm by 0.8mm, where the chitosan membrane was placed in test group and the dura mater was not removed in control group. Eight weeks (when the dura mater was considered reconstructed) after surgery, observation and histology test were run to find the difference between the two groups. Results Postoperative observation showed no swelling, no bleeding, and no fester; and the chitosan membrane was tightly wrapped with no adhesion and leakage. HE staining showed the new grown fiber membrane was mature with no bFGF nor EGF expression. Conclusion Chitosan membrane is effective in repairing dura mater, and the new fiber membrane is histologically close normal meninges.
