摘要
目的比较手术前后电解质,不同手术前后电解质变化,为围术期合理补充电解质提供依据。方法回顾性分析2014年3~7月在我院行手术治疗,术后转重症监护病房(ICU)治疗的患者340例。对所有患者手术前后电解质及不同手术前后电解质的变化进行分析。结果340例患者,其中168例(49.4%)患者术后血清钾浓度〈3.5mmol/L,82例(24.1%)患者术后血清钠浓度〈135mmol/L。318例(93.5%)患者术后血清钙浓度〈2.2mmol/L,113例(33.2%)患者术后血清镁浓度〈0.75mmol/L。各手术术后电解质与术前相比,差异有统计学意义(P〈0.05);体表手术与胸腹腔手术相比,手术前、后血清钙离子、镁离子差异有统计学意义(P〈0.05);甲状腺手术与乳腺癌手术相比,术后血清钾离子、磷离子差异有统计学意义(P〈0.05)。结论各手术术后各种电解质离子较术前明显降低;胸腹腔部手术前后均存在血清钙、镁离子浓度降低,乳腺癌手术术后血清钾离子浓度较其它手术降低明显,临床麻醉时及在麻醉恢复监护期间应予以高度重视。
Objective To compare the preoperative and postoperative electrolyte levels and electrolytes levels before and after surgery, and to provide a reasonable basis on perioperative added electrolyte amounts. Methods 340 patients who were at intensive care unit (ICU) after surgery from March to June 2014 in our hospital were analyzed retrospectively.. Electrolytes before and after surgery in all patients and different electrolyte changes before and after surgery were analyzed. Results In 340 patients, there were 168 cases (49.4%) whose serum potassium levels were lower than 3.5 mmol/ L, 82 eases (24.1% ) whose serum sodium concentration were lower than 135 mmol / L, 318 cases (93.5%)whose serum calcium concentration were lower than 2.2 mmol/ L, 113 cases (33.2%)whose serum magnesium concentration were lower than 0.75 mmol / L Electrolyte levels after the surgery compared with before surgery were statistically different(P〈0.05 ) ; Serum calcium and magnesium ions before surgery in Body surface surgery were significantly different from (P〈0.05) those in chest and abdomen cavity surgery; Postoperative serum potassium and phosphate ions in thyroid surgery were significantly different from those in breast cancer surgery (P〈0.05). Conclusion The electrolyte levels after operation were decreased compared with those before operation; Serum calcium and magnesium concentrations both decreased before and after chest and abdomen surgery. Serum potassium concentration after breast cancer surgery were significantly lower than that in other surgeries. All these should be given more attention in the clinical anesthesia.
