摘要
目的观察心脉隆注射液对慢性心力衰竭(CHF)合并室性早搏患者窦性心率震荡(HRT)的影响。方法选择心力衰竭合并室性早搏住院患者140名,随机分为两组,常规治疗组(A组)和心脉隆组(B组)各70例,两组患者在治疗前后均行24h动态心电图检查,计算并测量24h动态心电图中HRT的初始值(TO)、斜率(TS)和震荡斜率起始时间(TT),比较治疗前后两组患者HRT指标的差异。结果治疗2周后,心脉隆组患者HRT指标初始值(TO)和起始时间(TT)均较治疗前有明显下降,震荡斜率(TS)较治疗前明显上升(P<0.05);心率、室早次数较前明显下降(P<0.05);通过多元逐步回归分析发现,影响震荡初始的因素主要有心率、联律间期、室早次数;影响震荡斜率的主要因素有联律间期、年龄和室早次数(P<0.05)。结论心脉隆注射液可显著改善慢性心力衰竭合并室性早博患者的心脏自主神经功能,预防恶性室性心律失常的发生。
Objective To investigate the effect of Xinmailong injection on heart rate turbulence （HRT） in patients with chronic heart failure （CHF） complicated by ventricular premature beats. Methods A total of 140 hospitalized CHF patients with ventrieular premature beats were enrolled and randomly divided into conventional treatment group and Xinmailong group, with 70 patients in each group. The patients in both groups were given 24-hour ambulatory electrocardiography before and after treatment. The turbulence onset （TO）, turbulence slope （TS）, and turbulence timing （TF） of HRT on the 24-hour ambulatory electrocardiogram were measured and calculated, and the differences in HRT indices before and after treatment were compared between the two groups. Results After 2 weeks of treatment, the Xinmailong group had significant reductions in TO and TF and a significant increase in TS （P〈0.05）, as well as significant reductions in heart rate and the number of ventricular premature beats （P〈0.05）. The multiple stepwise regression analysis showed that heart rate, coupling interval, and number of ventricular premature beats were influencing factors for TO, and coupling interval, age, and number of ventricular premature beats were influencing factors for TS （P〈 0.05）. Conclusion In CHF patients with ventrieular premature beats, Ximnailong injection can significantly improve autonomic nervous function of the heart and prevent malignant ventricular arrhythmias.
