摘要
目的观察微创手术腔内激光联合药物治疗下肢淤积性皮炎的疗效。方法下肢淤积性皮炎患者43例,共60例肢体,分为试验组和对照组,每组各30例患肢。两组均接受药物治疗1个月;试验组除药物治疗外还联合腔内激光手术治疗。通过对比两组治疗前,接受治疗后2周及4周时皮损红斑、鳞屑、肿胀、色素沉着、渗液、毛发脱落、疼痛、瘙痒程度的评分,并记录两组出现并发症的情况,客观评价腔内激光联合药物治疗淤积性皮炎的疗效和不良反应。结果试验组治愈5例,显效21例,有效4例,有效率为86.7%;对照组治愈2例,显效14例,好转11例,无效3例,有效率为53.3%,经Ridit分析,P<0.05,两组比较差异有统计学意义。结论腔内激光联合药物治疗淤积性皮炎与单用药物治疗相比,具有更好疗效,但长期疗效尚需进一步观察。
Objective Lower-extremity stasis dermatitis is used to be treated by drugs, which seems only deal with the symptoms. This research aimed to evaluate the therapic effect of endovenous laser treatment(EVLT), a new kind of minimally invasive operation, in the treatment of stasis dermatitis. Methods A total of 43 patients with stasis dermatitis on 60 lower limbs were divided into two groups, with 30 limbs in experimental group and control group respectively. Both two groups were treated with oral and topic medicine for one month, the experimental group also accepted EVLT. The effect was assessed based on the changes from baseline to days 14 and 28 after the start of the therapy, the changes include erythema, scale, edema, pigmentation, exudation, hair loss, itching and pain of the affected area. In addition, the complications in the two groups were also compared. Results In experimental group, 5 patients were cured, while 21 patients were significantly improved and 4 patients were slightly improved, the effective rate was 86.7%. Meanwhile, in the control group, 2 patients were cured, while 14 patients were significantly improved and 11 patients were slightly improved, the effective rate was 53.7%. According to Ridit analysis, the therapeutic effect had statistic difference between the two groups(P〈0.05). Conclusion Comparing with simple drug therapy, EVLT could significantly improve the symptoms of stasis dermatitis, but further studies are needed for evaluating the long-term therapeutic effects of EVLT.
