摘要
目的：改进复用金属气管套管清洗消毒灭菌模式,纳入消毒供应中心集中管理,降低气管切开患者的感染风险,确保患者安全,为临床提供优质服务。方法从复用金属气管套管的配置上入手,将内套管配置从1个增加至5个,形成1拖5的规格,临床使用后由消毒供应中心按标准化流程进行清洗消毒灭菌处理并下送至临床科室。对金属气管套管的无菌性、功能完好性、操作便捷性、供应及时性4个方面进行跟踪量化评价并与传统方法进行对照。结果消毒供应中心集中管理复用金属气管套管1296套,更换内套管8732个。改进后金属气管套管内套管细菌培养合格率从93.65%提高至100.00%,功能完好性从98.24%提高到99.23%,操作便捷性从74.29%提高至100.00%,供应及时性从93．33%提高到98．57%,差异均有统计学意义（χ^2值分别为9.56,4.36,61.97,7．42；P〈0.01）。结论消毒供应中心集中管理复用金属气管套管的实践,达到灭菌质量、功能完好性、操作便捷性及供应及时性的有效提升,减少医院感染发生,提高工作效率,方便临床操作使用,解除临床科室后顾之忧的目的,充分发挥了消毒供应中心的职能和专业优势。
Objective To improve the supply mode of reusable metal tracheal tube, enhance the centralized management of sterilized serving center, reduce the risk of contagion of tracheotomy and improve the clinical service quality. Methods We changed the setting of the reusable metal tracheal tube, and the number of endotracheal tube were increased from 1 to 5. After clinical use, the tubes would be cleaned and sterilized by sterilization and supply center according to standard operating procedure, and then be sent to clinical departments. The sterility, function integrity, operating convenience and supply timeliness of metal trachea cannula shoule be tracked and evaluated, and be compared with the traditional method. Results Totally 1 296 sets of reusable metal tracheal tube were centralized managed by the sterilization and supply center, and 8 732 sets of endotracheal tubes had been changed. The qualified rate of bacteria cultivation in improved metal trachea casing tube increased from 93. 65% to 100%; the function integrity increased from 98. 24% to 99. 23%; the operating convenience increased from 74. 29% to 100%, the supply timely increased from 93. 33% to 98. 57%（χ^2=9.56,4.36,61.97,7. 42;P〈0.01） . Conclusions The practice of centralized management of reusing metal tracheal tube in the sterilization and supply center can normalize the cleaning and sterilization operation mode of the hospital; improve the sterilization quality, functional integrity, operation convenience and supply timely;reduce the occurrence of hospital infection; improve work efficiency; and made clinical operation more convenient. These actions give full play to the functions and professional advantages of steralization and supply center.
