实时三维经胸超声心动图在二尖瓣脱垂中的诊断应用 被引量:2

摘要 目的 探讨实时三维经胸超声心动图(RT3D-TTE)在诊断二尖瓣脱垂(MVP)中的应用价值。方法 选取我院自2013~2015年收治的20例MVP患者为研究对象,术前行RT3D-TTE检查,三维图像经过旋转及切割,确认二尖瓣膜脱垂区域,判断有无腱索断裂,与手术结果进行对照。结果 20例患者RT3D-TTE诊断二尖瓣脱垂与手术结果比较,前叶分区A1、A2、A3对比结果一致的病灶分别为5、8、5个,后叶分区P1、P2、P3对比结果一致的病灶分别为4、6、6个。RT3D-TTE诊断腱索断裂与手术结果一致共9例,RT3D-TTE诊断腱索断裂但术中未发现腱索断裂2例;RT3D-TTE与手术所见诊断无腱索断裂一致9例。结论 RT3D-TTE可快速、准确、高效地评价MVP,为制定适宜的手术方案提供可靠依据。 Objective To investigate the application value of real-time three-dimensional transthoracic echocardiography(RT3D-TTE) in the diagnosis of mitral valve prolapse(MVP). Methods 20 patients with MVP who were admitted to our hospital from 2013 to 2015 were selected as subjects. Preoperative RT3D-TTE examination was carried out. Threedimensional images were rotated and cut to confirm the mitral valve prolapse area, and determine whether the chordae rupture occurred. The surgical results were compared. Results The results of RT3D-TTE diagnosis in 20 patients with mitral valve prolapse were compared with the surgical results in the anterior leaflets A1, A2, A3, and the number of cases with consistent results were 5, 8, and 5 respectively. RT3D-TTE diagnosis of mitral valve prolapse was compared with the surgical results in the posterior leaflets P1, P2, P3, and the number of cases with consistent results were 4, 6,and 6 respectively. RT3D-TTE diagnosis of chordae rupture and surgical results were consistent in 9 cases, RT3D-TTE diagnosis of chordae rupture but no chordae rupture during surgery were in 2 cases; RT3D-TTE diagnosis and no chordae rupture observed in the surgery were consistent in 9 cases. Conclusion RT3D-TTE can be used to evaluate MVP quickly, accurately and effectively, which can provide a reliable basis for the development of appropriate surgical procedures.
