摘要
目的探讨不同氟喹诺酮类药物治疗老年社区获得性肺炎的临床观察及副作用。方法 选择杭州市红十字会医院2011年5月~2014年5月收治的197例老年社区获得性肺炎患者作为研究对象,分为莫西沙星组96例、环丙沙星组56例和左氧氟沙星组45例,研究对象除抗菌药物治疗不同,其他药物均相同。对比三组临床疗效、细菌清除率、不良反应。结果 莫西沙星组、环丙沙星组、左氧氟沙星组临床有效率分别为88.5%、76.8%、71.1%。莫西沙星组与环丙沙星、左氧氟沙星组有效率比较,差异有统计学意义（χ^2=6.2、10.7,P〈0.05）。环丙沙星、左氧氟沙星组有效率比较,差异无统计学意义（χ^2=0.7,P〉0.05）。莫西沙星组、环丙沙星组、左氧氟沙星组细菌清除率分别为87.5%、71.4%、66.7%。三组比较,莫西沙星组细菌清除率最高（P〈0.05）,环丙沙星、左氧氟沙星组细菌清除率比较,差异无统计学意义（P〉0.05）。莫西沙星组、环丙沙星组、左氧氟沙星组不良反应发生率分别为6.3%、5.4%、4.4%,三组差异无统计学意义（P〉0.05）。结论 与左氧氟沙星、环丙沙星比较,莫西沙星治疗老年社区获得性肺炎有更好的有效率及细菌清除率。
Objective To explore the clinical observation and side effects of different fluoroquinolones in the treatment of community acquired pneumonia in the elderly. Methods A total of 197 elderly patients with community acquired pneumonia admitted in Hangzhou Red Cross Hospital from May 2011 to May 2014 were enrolled in the study. They were divided into moxifloxacin group（n=96）, ciprofloxacin group（n=56） and levofloxacin group（n=45）. All subjects were treated with the same drugs except the antibacterial agents. The clinical efficacy, bacterial clearance and adverse reactions were compared among the three groups. Results The clinical effective rates of moxifloxacin group, ciprofloxacin group and levofloxacin group were 88.5%, 76.8% and 71.1%, respectively. The difference in the efficiency between moxifloxacin group and ciprofloxacin, levofloxacin group was statistically significant（χ^2=6.2 and 10.7, respectively, P〈0.05）. While there was no statistically significant difference in the efficiency between ciprofloxacin group and levofloxacin group（χ^2=0.7, P〉0.05）. The bacterial clearance rates of moxifloxacin group, ciprofloxacin group and levofloxacin group were 87.5%, 71.4% and 66.7%, respectively, with the bacterial clearance rate of moxifloxacin group as the highest（P〈0.05）.There was no significant difference in bacterial clearance rate between ciprofloxacin and levofloxacin groups（P〈0.05）.The incidences of adverse reactions in the moxifloxacin group, ciprofloxacin group and levofloxacin group were 6.3%,5.4% and 4.4%, respectively, with no statistically difference between the three groups（P〉0.05）. Conclusion Moxifloxacin in the elderly community-acquired pneumonia has a better efficiency and bacterial clearance rate compared with levofloxacin, ciprofloxacin.
