CT引导臭氧造影在多节段腰椎间盘退变诊治中的运用

摘要 目的:探讨CT引导下臭氧腰椎间盘造影在腰椎间盘多节段退变性疾病的临床诊疗价值。方法:选取经保守治疗6个月症状无缓解的多节段腰椎间盘退变疾病的患者90例,随机分为两组,椎间盘造影组对术前经MRI显示位腰椎间盘退变的节段行CT引导椎间盘造影术,根据造影结果选择手术治疗的节段;对照组根据影像结果及患者体征选择手术治疗的节段。结果:随访及比较两组患者术后3个月,6个月,1年后VAS评分及ODI指数均较术前明显好转,但VAS评分ODI指数臭氧腰椎间盘造影优于对比组。结论:腰椎间盘多节段退变性患者术前依据椎间盘臭氧造影术后纤维环不同的影像形态,指导临床手术治疗方案,可有效减少需手术干预节段,临床疗效得到了提高,且减少了术后的疼痛反应,具有临床应用价值。 Objective To explore of clinical application of gas discography in diagnosis and treatment of multi-segmental lumbar degenerative diseases. Method A total 90 patients with multi-segmental lumbar degenerative diseases by low back pain were divided into 2 groups. Discography group underwent gas discography examination and conventional radiographic examination;based on the finding the selected segments were subjected to surgical intervention. Conwentional treatment group only receive routine imaging examination,and surgical intervention segments were chosen accordingly. The patients were followed up and ODI and VAS scores were compared beween the 2 groups preoperatively and at postoperative 3 months,6 months and 12 months. Results Compared with those pre-operation,the ODI and VAS scores of both groups were improved. The follow-up results also showed that the ODI and VAS scores in gas discography group were significantly superior to those of the conventional treatment group. Conclusion Gas discography has a good clinical value for multi-segmental lumbar degenerative diseases patients with obious low back pain. Multi segment degenerative lumbar disc patients before surgery based on the disc annulus different image form ozone postoperative angiography,clinical surgical treatment,which can effectively reduce the need for surgical intervention segments,clinical efficacy has been improved,and reduced postoperative pain response,has clinical value.
