摘要 目的探讨肿瘤放疗对患者的甲状腺功能的损伤情况。方法整群选取该院2014年1月—2016年1月需要接受放疗的肿瘤患者共108例,分别观察和记录所有患者未接受放疗、接受放疗后、接受放疗180 d后、接受放疗360 d后的甲状腺功能指标,包括甲状腺过氧化物酶,甲状腺球蛋白、游离四碘原氨酸、游离三碘甲状腺原氨酸、促甲状腺激素。并比较患者在这4个时间段的临床症状。结果未放疗时,患者的游离四碘原氨酸平均值为(11.94±4.05)nmol/L、游离三碘甲状腺原氨酸平均值为(5.76±2.23)nmol/L,促甲状腺激素平均值为(3.77±1.45)m IU/L;放疗180 d后,患者的游离四碘原氨酸平均值为(9.58±3.32)nmol/L、游离三碘甲状腺原氨酸平均值为(4.82±2.02)nmol/L,促甲状腺激素平均值为(4.50±2.07)m IU/L;放疗360 d后,患者的游离四碘原氨酸平均值为(8.61±2.78)nmol/L、游离三碘甲状腺原氨酸平均值为(4.12±1.72)nmol/L,促甲状腺激素平均值为(5.34±2.89)m IU/L,与未放疗相比差异有统计学意义(P<0.05)。放疗180 d后,所有患者并未出现相应的甲减(甲状腺功能减退)临床症状。放疗360 d后,有7例患者出现了甲减临床症状,包括说话行动迟缓、头晕失眠、食欲减退、乏力、精神不振,经治疗后症状缓解。结论肿瘤放疗对甲状腺功能确有损伤,但是损伤程度较轻,不会出现特异性临床症状。接受肿瘤放疗的患者应该定期检测甲状腺功能,如有必要要采取相应的治疗措施。 Objective To investigate the thyroid function damage in patients with tumor radiotherapy. Methods Group selection our hospital from January 2014 to January 2016 to accept tumor radiotherapy in patients with a total of 108 cases were observed and recorded in all of the patients did not receive radiotherapy, after radiotherapy, radiotherapy, radiotherapy180 days after 360 days of thyroid function, including thyroid peroxidase, thyroglobulin, free triiodothyronine, free four iodine three triiodothyronine, thyroid stimulating hormone. The clinical symptoms of patients in these four periods were compared. Results Without radiotherapy, patients with free four iodine originalacid average(11.94 ±4.05)nmol/L,You Lisan triiodothyronine average(5.76±2.23) nmol/L, TSH average(3.77±1.45) m IU/L radiotherapy; 180 days later, with free four iodine originalacid flat the average(9.58±3.32) nmol/L, free triiodothyronine three average(4.82±2.02) nmol/L, TSH average(4.50±2.07) m IU/L radiotherapy; 360 days later, with free four iodine originalacid average(8.61 ±2.78) nmol/L, free three iodine the thyroid gland originalacid average(4.12 ±1.72) nmol/L, TSH average(5.34 ±2.89) m IU/L, compared with no radiation,the difference was statistically significant(P〈 0.05). 180 days after radiotherapy, all patients did not appear in the corresponding hypothyroidism(hypothyroidism) clinical symptoms. 360 days after radiotherapy, 7 patients had clinical symptoms of hypothyroidism, including talk slow, insomnia, loss of appetite, dizziness, fatigue, lethargy, relief of symptoms after treatment. Conclusion Radiotherapy has some damage to thyroid function, but the degree of injury is mild. Patients receiving radiotherapy should regularly detect thyroid function and take appropriate treatment if necessary.
