摘要
目的探讨针灸背俞穴联合局部围刺治疗黄褐斑的疗效。方法将96例黄褐斑患者随机分为联合组、针灸组和围刺组各32例。针灸组给予单纯针灸背俞穴治疗,围刺组给予单纯局部围刺治疗,联合组给予针灸背俞穴联合局部围刺治疗。观察3组治疗前后中医证候积分及血清超氧化物歧化酶(SOD)、丙二醛(MDA)水平变化情况,统计3组临床疗效、不良反应及复发情况。结果治疗后3组中医证候积分均显著降低(P均<0.05),且联合组明显低于针灸组和围刺组(P均<0.05);联合组总有效率明显高于针灸组和围刺组(P均<0.05);治疗后联合组血清SOD和MDA水平均显著改善(P均<0.05),针灸组和围刺组SOD水平显著改善(P均<0.05),且治疗后联合组SOD和MDA水平改善情况均显著优于针灸组和围刺组(P均<0.05);3组均未发生不良反应,且3组随访期间复发率比较差异均无统计学意义(P均>0.05)。结论针灸背俞穴联合局部围刺治疗黄褐斑较单纯针灸和围刺疗效更为显著,推测和改善血清SOD和MDA水平存在一定关联。
Objective It is to study the effect of acupuncture on Shenshu point back joint with local peripherl acupuncture on chloasma. Methods 96 cases of patients with chloasma were randomly divided into combination group treated with acupuncture on Shenshu point back joint with local peripherl acupuncture, acupuncture group treated with acupuncture on Shenshu point and peripherl acupuncture group treated with local peripherl acupuncture, each group had 32 cases. The changes of traditional Chinese medicine syndrome scores and levels of superoxide disproportionation enzyme （ SOD ）,malondialdehyde （MDA） in serum before and after treatment in the three groups were observed, the clinical curative effect, adverse reaction and recurrence were recorded and compared. Results After treatment TCM syndrome scores were significantly decreased in the three groups（P 〈 0. 05 ）,and the decrease in combination group was more significant than that in the other two groups （P 〈 0. 05 ） . The total effective rate in combination group was higher than that in the other two groups （ P〈 0. 05 ） . The levels of serum SOD and MDA in combination group were significantly improved （ P〈 0. 05 ） , and the improvements were better than that in acupuncture group and peripherl acupuncture group （P〈0.05） , that of SOD was improved in acupuncture group and peripherl group （P〈0. 05） . No adverse reaction was found in the three groups, and there was no difference in the recurrence rate during following-up period among the three groups（P 〉 0. 05） . Conclusion Acupuncture on Shenshu point back joint with local peripherl acupuncture has better curative effect on chloasma, improving serum SOD and MDA may be one of the mechanisms.
