Diagnostic Value of Ventana-IHC for ALK+ Non-small Cell Lung Cancer:A Meta-analysis

摘要 [目的]系统评价Ventana-IHC对ALK+非小细胞肺癌(NSCLC)诊断的准确性及临床应用价值。[方法]检索Pub Med、Embase、Cochrane Library、中国生物医学文献数据库、中国知网、万方数据库和中文科技期刊全文数据库(维普数据库),收集Ventana-IHC用于诊断ALK+NSCLC的研究数据。通过Meta分析拟合受试者工作特征曲线(summary receiver operating characteristic,SROC),合并诊断效应量,评估其诊断效能。[结果 ]Meta分析共纳入9项研究,总的病例数为2041例。Ventana-IHC对ALK+NSCLC诊断的合并敏感性和特异性分别为0.94(95%CI:0.90~0.97)和1.00(95%CI:0.99~1.00),合并阳性似然比(positive likelihood ratio,PLR)和阴性似然比(negative likelihood ratio,NLR)分别为75.99(95%CI:42.91~134.56)和0.10(95%CI:0.04~0.22),合并诊断比值比(diagnostic odds ratio,DOR)为950.48(95%CI:399.11~2263.54)。SROC曲线显示,曲线下面积(area under curve,AUC)为0.9962(SEAUC 0.0041),Q*统计量为0.9766(SEQ*0.0149)。[结论]Ventana-IHC对ALK+NSCLC诊断的敏感度较高,特异性高,在某种程度上可替代FISH检测用于对ALK+NSCLC的检测和诊断,具有一定的临床应用价值。 [Purpose] To evaluate the diagnostic value of Ventana-IHC for ALK+ non-small cell lung cancer(NSCLC) by Meta-analysis. [Methods] All required English and Chinese literatures in Pubmed,Embase,Cochrane Library,Chinese Biological Medical Literature Database,China National Knowledge Infrastructure,Wanfang and VIP database were searched. Then further screening based on the inclusion and exclusion criteria. Meta-analysis of diagnostic test was applied to analyze the sensitivity and specificity of Ventana-IHC for the detection of ALK + NSCLC. [Results] Nine articles were included in the Meta-analysis,which included2041 samples. The sensitivity,specificity,positive likelihood ratio(PLR),negative likelihood ratio(NLR) and diagnostic odds ratio(DOR) of Ventana-IHC in the diagnosis of ALK+ NSCLC were 0.94(95%CI:0.90-0.97),1.00(95%CI:0.99-1.00),75.99(95%CI:42.91-134.56),0.10(95% CI:0.04 -0.22),950.48(95% CI:399.11 -2263.54),respectively. The area under curve(AUC) of summary receiver operating characteristic(SROC) was 0.9962(SEAUC 0.0041). The Q* value was 0.9766(SEQ* 0.0149). [Conclusion] The specificity and sensitivity of VentanaIHC were relatively high. In a certain degree,Ventana-IHC could be substituted for FISH to be used in the diagnosis of ALK+NSCLC with clinical practical value.
