Relationship Single Nueleotide Polymorphisms of GNAS1 T393C and Prognosis of Advanced Non-small Cell Lung Cancer

摘要 [目的]分析GNAS1 T393C多态性与晚期肺癌患者总生存率的相关性。[方法]选取2010年3月至2012年3月94例经病理证实Ⅲ~Ⅳ期接受放疗或化疗的肺癌患者。提取患者全血基因组DNA,应用PCR扩增目的基因片段,Sanger双脱氧测序方法分析GNAS1T393C位点的单核酸多态性基因型。应用SPSS12.0软件COX生存分析临床因素及基因型与晚期肺癌患者生存的相关性。[结果]随访至2012年3月,94例肺癌患者中位生存期14个月,1年、2年总生存率分别为61%和35%。COX生存分析显示GNAS1 T393C三种基因型TT、TC和CC间生存率有统计学差异(OR=0.660,95%CI:0.051~8.568,P=0.039);携带等位基因C的患者生存率明显低于等位基因T携带者(OR=0.333,95%Cl:0.026~4.284,P=0.030)。[结论 ]GNAS1 T393C多态性与肺癌总生存率相关,携带等位基因C的肺癌患者预后更差。 [Objective] To explore the relationship between single nueleotide polymorphisms(SNP)of GNAS1 T393 C and overall survival of advanced lung cancer. [Methods] Between March 2010 and March 2012,genomic DNA was extracted from whole blood in 94 cases with advanced lung cancer patients proven pathology in this study. Allelic discrimination of GNAS1 was performed by quantitative real-time polymerase chain reaction. Genetyping was correlated with survival of advanced lung cancer. The Kaplan-Meier method and log-rank test were used to assess the prognostic significance of the clinical factors and genotypes for survival. Cox forward-stepwise regression model was used to assess genetic risk factors in multivariate analyses. [Results] Followed up to March 2012,the median survival of 94 cases with lung cancer was 14 months;the 1-,2-year ovrall survival was 61% and 35% respectively. Cox analysis showed that the overall survival received benefit from TT genetype of GNAS1 T393C(OR=0.660,95%CI:0.051~8.568,P=0.039),patients with allele T carrier in GNAS1 T393 C gene had a worse prognosis(OR=0.333,95%CI:0.026~4.284,P=0.030). [Conclusions] Genetic polymorphism in the GNAS1 T393 C is related to prognosis in advanced lung cancer,which may be a prognostic factor.
