微量白蛋白尿水平与慢性心力衰竭的关系 被引量:1

Correlation of Microalbum inuria and Cardiac Functional Classification of Chronic Heart Failure

摘要 目的探讨微量白蛋白尿(MAU)与慢性心力衰竭(CHF)患者心功能之间的关系。方法对2015年1月-2016年1月期间在武清区人民医院接受治疗的100例出现蛋白尿的CHF患者的病历资料进行回顾性研究。根据患者心功能的不同将其分为心功能Ⅱ级组(33例)、心功能Ⅲ级组(39例)和心功能IV级组(28例)。同时选取同一时期在武清区人民医院体检正常者40例为对照组。测定并比较4组的MAU、脑钠肽(BNP),另对心功能分级与MAU进行相关性分析。结果 BNP水平比较心功能Ⅳ级组>Ⅲ级组>Ⅱ级组>对照组差异有统计学意义(P<0.05)尿液中白蛋白的含量比较,心功能Ⅳ级组>Ⅲ级组>Ⅱ级组>对照组差异有统计学意义(P<0.05)。MAU水平与CHF患者的心功能分级具有相关性(r=0.59,P<0.05)。结论 CHF患者可出现白蛋白尿的情况而且MAU与CHF的严重程度密切相关。 Objective To investigate the relationship between the level of microalbum inuria(MAU) and cardiac functional classification of chronic heart failure(CHF).Methods 100 patients with CHF were selected in Wuqing People's Hospital from January 2015 to January 2016.They were divided into Heart function Ⅱ Level group with 33 examples,Heart function Ⅲ Level group with 39 examples;Heart function Ⅳ level group with 28 examples according to NYHA standards.A total of 40 cases with normal physical examination were selected as control group at the same period.MAU,brain natriuretic peptide(BNP) were measured and compared among the four groups.The correlation of cardiac function classification and MAU were analyzed.Results The BNP levels in four groups were compared that cardiac function level Ⅳ group level Ⅱ group level Ⅲ group control group,and the difference was statistically significant(P〈0.05);The MAU content in four groups were compared that cardiac function level Ⅳ group level Ⅱ group level Ⅲ group control group,and the difference was statistically significant(P〈0.05);The level of MAU was positively correlated with the grade of nonconcentric function for CHF(r=0.59,P〈0.05).Conclusion Patients with CHF may have proteinuria and show that the severity of heart failure is closely related to MAU.
