摘要
目的探讨腹腔镜根治术在进展期胃癌中的临床疗效及对细胞免疫功能的影响,评估其临床价值。方法选取我院2012年1月~2014年10月收诊的180例进展期胃癌患者作为研究对象,随机分为观察组与对照组,各90例。对照组给予传统开腹术治疗,观察组给予腹腔镜根治术治疗,比较两组的疗效以及术后免疫功能指标。结果观察组的手术时间长于对照组,术后住院时间、术后排气时间、切口长度短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。两组的清扫淋巴节数量比较,差异无统计学意义(P>0.05)。两组术后的CD3+、CD4+、CD8+、CRP和IL-6水平显著优于术前,差异有统计学意义(P<0.05)。观察组术后3、7、12 d的CD3+、CD4+、CD8+水平显著高于对照组同一时间段,CRP和IL-6水平显著低于对照组同一时间段,差异有统计学意义(P<0.05)。结论腹腔镜根治术在进展期胃癌中的临床疗效较传统开腹术疗法明显,能够减少对患者细胞免疫功能的影响,值得临床推广应用。
Objective To explore the clinical efficacy of laparoscopic radical gastrectomy in advanced gastric cancer and its influence on cellular immune function,and to evaluate its clinical Value.Methods 180 patients with advanced gastric cancer in our hospital from January 2012 to October 2014 were selected and randomly divided into the observa- tion group and the control group,90 cases in each group.The control group was treated with traditional open surgery,and the observation group was treated with laparoscopic radical operation.The efficacy and postoperative immune function index of the two groups were compared.Results The operation tinw oLthe observation group was longer than that of the control grotip,the postoperative hospital stay,postoperative exhaust lime,incision length of the observation group was shorter than that of the control group,and the amount of intraoperative bleeding of the observation group was less than that of the control group,with significant difference （P〈O.05）.There was no significant difference between the two groups in the number of dissection lymph nodes （P〉0.05）.The level of CD3＋,CD4＋,CD8＋,CRP and IL-6 after operation in the two groups was better than that before operation,with significant difference （P〈0.05）.The level of CD3＋,CD4＋,CD8＋after operation at 3,7,12 days in＇he observation group was~ higher than that in the control group at the same time,the level of CRP and IL-6 after operation at 3,7,12 days in the observation group was lower than that in the control group at the same time,with significant difference （P〈0.05）.Conclusion The clinical effect of laparoscopic radical operation in ad- vanced gastric cancer is more significant than that of traditional open surgery,can reduce the influence on the patients cellular immune function,and it is worth of clinical promotion and application.
