摘要
目的:探讨动脉内留置导管持续灌注化疗联合胃左/胃右动脉栓塞术治疗中晚期胃底-贲门癌的应用价值及临床效果。方法:选取我院79例中晚期胃底-贲门癌,男48例,女31例。经股动脉穿刺,采用Seldinger技术经股动脉插管将导管置于腹腔动脉超选胃左动脉、胃右动脉,进行血管造影。动脉内留置导管微量泵灌注化疗药物表阿霉素(EADM)、丝裂霉素(MMC)、奥沙利铂(OXA)、5-氟尿嘧啶(5-FU),再使用超液化碘(UFL)与表阿霉素(EADM)混合成碘油乳剂+明胶海绵颗粒行胃左动脉栓塞术。结果:近期有效率显著提高,复查病例中原发病灶、区域淋巴结转移及转移灶有不同程度减小,临床症状减轻,全组未发生严重副作用及并发症。完全缓解(CR)16例、部分缓解(PR)47例、稳定(SD)22例、病灶进展(PD)4例。结论:动脉内留置导管持续灌注化疗联合栓塞术治疗中晚期胃底-贲门癌能改善患者的生存质量,提高生存率,是治疗中晚期胃底-贲门癌的有效方法。
Objective：To evaluate the clinical value of intra-arterial catheter infusion chemotherapy combined with gastric left/or right gastric artery embolization in the treatment of advanced gastric fundus-cardia carcinoma.Methods：79cases of advanced gastric fundus- cardia cancer,including 48 males and31females were conducted in this study.The femoral arterial puncture was performed and the catheter was placed on the left gastric artery and the right gastric artery of the celiac artery via Seldinger technique to perform angiography.（EADM）,mitomycin C（MMC）,oxaliplatin（OXA）,5-fluorouracil（5-FU）and intra-arterial indwelling catheter infusion of chemotherapeutic drugs were used.And epirubicin（EADM）mixed into lipiodol emulsion and gelatin sponge particles were used to perform left gastric artery embolization.Results：The effective rate was significantly improved.The primary lesion,regional lymph node metastasis and metastatic foci were reduced and the clinical symptoms were relieved.There was no serious side effect or complication in all of the groups.There were showed complete remission（CR）in 16 cases,partial remission（PR）in 47 cases,stable（SD）in 22 cases,lesion progression（PD）in 4cases.Conclusion：Intra-arterial indwelling catheter continuous infusion chemotherapy combined with embolization in the treatment of advanced gastric fundic-cardia cancer can improve the quality of life of patients and improve survival rate,is a effective method for the treatment of advanced gastric fundus-cardia cancer.
