摘要
目的比较股骨近端锁定加压接骨板与股骨近端防旋髓内钉治疗老年股骨近端不稳定性骨折的临床疗效。方法回顾分析我科2009年9月~2014年9月收治的60例老年股骨近端不稳定性骨折患者,其中28例患者采用股骨近端锁定加压接骨板（proximal femoral locking compression plate,PF-LCP）内固定（PF-LCP组）,32例采用股骨近端防旋髓内钉（proximal femoral nail antirotation,PFNA）内固定（PFNA组）,比较两组患者切口长度、手术时间、术中出血量、术后并发症、骨折愈合时间及术后髋关节Harris评分。结果与PF-LCP组相比,PFNA组患者术中出血量较PL-LCP组患者明显要少,手术切口长度、手术时间、骨折愈合时间较PL-LCP组患者明显要短,两者都有统计学差异（P〈0.05）;两组患者术后并发症及髋关节Harris评分无统计学差异（P〉0.05）。结论与PFLCP相比,PFNA在股骨近端不稳定性骨折的治疗中有助于缩短手术时间,减少手术创伤,缩短骨折愈合时间,尤其适用于老年骨质疏松患者。
Objective Comparison the clinical efficacy between proximal femoral locking compression plate and proximal femoral antirotation in the treatment of unstable femoral proximal fracture in elderly patients. Methods A retrospective analysis of 60 elder patients with unstable femoral proximal fracture treated by proximal femoral locking compression plate internal fixation（ PF-LCP group） or proximal femoral nail antirotation internal fixation（ PFNA group） in our department from September 2009 to September 2014. Twenty-eight patients were treated with PF-LCP,and the other thirty-two patients were treated with PFNA. The incision length,operation time,blood loss,postoperative complications,fracture healing time,and the Harris hip score postoperative of the patients have been evaluated. Results Compared with the PF-LCP group,the PFNA group significantly reduced the blood loss,incision length,operation time,fracture healing time,postoperative complications.（ P 0. 05）,although there were no significant differences in postoperative complications and the Harris hip score postoperative.（ P 0. 05）. Conclusion PFNA internal fixation can shorten the operation time,reduce the surgical trauma and the fracture healing time in the treatment of unstable proximal femoral fractures compared with PF-LCP internal fixation,especially for the elder patients with osteoporosis.
