摘要
目的通过在兔腹部复制缺损创面并于创基放置硅胶垫片,用背阔肌肌皮瓣带蒂移植进行修复,构建一种新型的肌皮瓣带蒂移植模型。方法采用新西兰大耳白兔20只,在兔腹部设计软组织缺损创面约6.0 cm×8.0 cm,创基覆盖硅胶垫片,背阔肌肌皮瓣带蒂转移修复腹部创面。按移植术后时间的不同进行分组,每组5只。观察肌皮瓣的大体存活情况、肌纤维的组织病理学改变及肌瓣中微血管密度的改变。结果大体观察各组肌皮瓣均存活良好,无皮瓣坏死,取材时见肌肉外观红润。HE及Masson染色结果显示各组肌纤维形态良好。各组间微血管计数6周>5周>4周>3周,各组内微血管计数近端>中段>远端,差异有统计学意义(P<0.05)。结论以胸背血管为蒂的兔背阔肌肌皮瓣移植修复腹部创面是一种理想的肌皮瓣移植模型,适用于药物、细胞及血运重建等实验研究。
Objective To reconstruct a new type of musculocutaneous flap pedicle graft by replanting the defect in the abdomen of rabbits and placing silicone gaskets in the wound. Methods Twenty New Zealand rabbits were chosen randomly as experimental animals. A 6.0 cm × 8.0 cm abdominal wound area was designed in rabbit abdomen and covered with silicon gasket in the surface of the wound to simulate poor blood circulation, and the latissimus dorsi muscle flap pedicled to repair the abdominal wound. The animals were randomly divided into 4 groups according to the transplantation time, 5 rabbits in each group. Musculocutaneous flap survival rate, histopathological changes of muscle fibers and micro vessel density changes in the musculoeutaneous flap were evaluated. Results All museulocutaneous flaps in all four groups survived well. There was no flap necrosis and the muscular tissue appeared ruddy. HE and Masson staining show that the morphology of the muscle fiber is sound. The micro vessel density was different between groups (6 w 〉5 w 〉4 w 〉3 w) and in each group (proximal 〉 middle 〉 distal), the difference was statistically significant. Conclusion Thoracodorsal artery pedicled rabbit latissimus dorsi musculoeutaneous flap transplantation was a feasible model for musculocutaneous flap transplantation. This model will be especially suitable in the experimental study of drugs, cells and neovascularization.
