摘要
目的探讨应用脉冲冲洗枪进行乳房注射物聚丙烯酰胺水凝胶(PAHG)冲洗的方法。方法自2013年8月至2015年10月,共进行了乳房PAHG注射物取出21例。常规组是采用乳房下皱襞切口,分离至注射腔隙,剥离形成的包块,去除变性组织,用冲洗球抽取含8万U庆大霉素的生理盐水3000 nl冲洗,将残留在腔隙内的盐水挤出。脉冲冲洗枪方法前部分与常规方法相同,后部分采用脉冲枪,用3000 ml盐水冲洗,利用脉冲枪的负压将冲洗液吸走。结果脉冲冲洗枪冲洗后,组织腔内残留的PAHG更少,冲洗液更清亮,包括感染患者在内,无再次感染发生。常规组残留的PAHG比冲洗枪组多,有1例患者出现了术后感染。结论脉冲冲洗枪能更好地清除乳房腔隙内残留的PAHG。
Objective Mammoplasty breast augmentation with polyacrylamide hydrogel （PAHG） has been employed for more than one decade. Many complications occurred during this time. Concerned about these complications, patients who accepted PAHG injections demanded removal of this substance. However, this substance is difficult to remove because of its adhesive character. Pulsed lavage is a common method of debridement. We removed PAHG by pulsed lavage in order to diminish residue of PAHG. Methods From Aug. 2013 to Oct. 2015, a total of 21 patients underwent this operation. We separated the patients into two groups. Thirteen pa- tients, including 3 patients with infection, underwent conventional methods with rinsing after surgery, the other underwent pulse lavage. All patients underwent normal steps to remove tissues which were damaged by infection. For the conventional group, 3000 ml normal saline was used, including 80 000 U gentamicin to rinse the lacuna. For the pulse group, this process was automated by flushing gun. The flushing fluid was simultaneously suctioned by the flushing gun. Results There was little PAHG residue after being flushed by the flushing gun. The resid- ual flushing fluid was clear, even in patients where infection was present. Among this group, no cases of infection remained. There was more PAHG residue in the conventional group than that in the group treated by the flushing gun. One case of infection remained after surgery. Conclusion Irrigating by pulsed lavage can remove PAHG more effectively than the conventional method.
