摘要
目的:探讨B超在鉴别宫内早孕囊、假孕囊与空孕囊中的临床意义。方法:选择2013年4月至2015年4月需要准确诊断妊娠的对象69例,经过手术或者临床确诊为空孕囊、早孕囊、或假孕囊,分别作为早孕组、空孕囊组以及假孕囊组,每组23例。采用彩色多普勒超声仪,分别进行经腹探查与经阴道探查,观察孕囊壁厚度、孕囊直径以及超声双环征以及卵黄囊情况。结果:早孕组患者囊壁轮廓光滑,回声较强且均一,厚度>2 mm,而另外两组患者的囊壁轮廓不规则且有缺损,厚度<1.2 mm;早孕组患者的孕囊平均直径与孕周数密切相关,随着孕周数增长,孕囊直径也同时增长(P<0.05),而假孕囊组患者在不同孕周期间,孕囊并无明显的变化(P>0.05),空孕囊组患者的孕囊直径虽然随着孕周的变化而变化,但是在9~10周时的孕囊直径小于早孕组(P<0.05)。结论:B超鉴别宫内早孕囊、假孕囊与空孕囊,安全性好、准确率高,是指导医护人员有效处理早期妊娠的重要依据。
Objective ： To explore the clinical significance of B - ultrasound in differential diagnosis of intrauterine pregnancy, pseudopregnancy and empty pregnancy sac. Methods ：69 cases by surgery or clinical confirmation from April 2013 to April 2015 were selected as pregnancy group, empty pregnancy group and pseudopregnancy group, 23 cases in each group. Color Doppler ultrasonography was adopted to make transabdominal and transvaginal examinations to observe the patients endometrial thickness, gestational sac diameter, ultrasonic double ring echo, and yolk sac state. Results：Embryo sac wall （ 〉2 mm） in the pregnancy group is smooth with strong and uniform echo while the embryo sac walls （ 〈 1.2 mm） in the other two groups were irregular with defects. The average diameters of the gestational sac in the pregnancy group were closely related to the gestational age ; the average diameter of the gestational sac was significantly increased with the growth of pregnancy weeks（ P 〈 0.05 ）. The average diameters of the gestational sac in the pseudopregnancy group was not significantly changed with the growth of pregnancy weeks（ P 〉 0.05 ）. The average diameter of the gestational sac in the empty pregnancy group changed with the pregnancy week but was less than that in the pregnancy group in the 9 - 10th week （ P 〈 0.05 ）. Conclusion： The adoption of B - ultrasound is safe and accurate in the diagnosis of pregnancy, pseudopregnancy and empty pregnancy, and provides an important basis in the effective treatment
