摘要
[目的]探讨机械辅助排痰联合足太阳膀胱经穴位按摩在痰浊壅肺型慢性阻塞性肺疾病急性加重期(AECOPD)病人护理中的应用效果。[方法]选取2014年7月—2015年12月南京中医药大学南通附属医院呼吸科收治的痰浊壅肺型AECOPD病人60例,随机分为试验组和对照组各30例。对照组在常规治疗护理基础上加机械辅助排痰,试验组在此基础上联合足太阳膀胱经穴位按摩,两组疗程均为7d。观察两组病人每天排痰量、治疗前后生命体征:[心率(HR)、呼吸(R)、经皮氧饱和度(SpO_2)]及动脉血气分析指标:[酸碱度(pH值)、动脉血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)]的变化。[结果]试验组排痰效果优于对照组,治疗后心率、呼吸频率优于对照组,SpO_2、PaO_2、pH值高于对照组,PaCO_2低于对照组。[结论]机械辅助排痰联合足太阳膀胱经穴位按摩能更有效地促进排痰,改善通气和病人血气指标,临床疗效优于单纯机械辅助排痰。
Objective：To probe into the application effect of Mechanical-assisted expectoration combined with foot sun bladder meridian acupoint massage in nursing care of acute exacerbation of phlegm obstructing lung type of chronic obstructive pulmonary disease patients（AECOPD）.Methods：A total of 60 cases of phlegm obstructing lung type of COPD patients were selected in department of respiration in Nantong affiliated hospital of Nanjing university of Chinese medicine from July 2014 to December 2015 and they were randomly divided into experimental group and control group,30 cases in each.Patients in control group were given conventional treatment combined with Mechanical-assisted expectoration.Besides that,the patients in experimental group received foot sun bladder meridian acupoint massage.The patients in two groups were treated for 7days.Two groups of patients were observed on daily sputum volume,vital signs before and after treatment including heart rate（HR,respiratory（R）,and percutaneous oxygen saturation（SpO2）and indicators of the arterial blood-gas analysis including degree of acid and alkaline（pH value）,arterial oxygen pressure（PaO2）and arterial blood carbon dioxide partial pressure（PaCO2）.Results：The expectoration effect in experimental group was better than that in control group.After the treatment,the HR and respiratory rate were better than those in control group,SpO2,PaO2 and PH value were higher than those in control group,and PaCO2 was lower than that in control group.Conclusion：Mechanical-assisted expectoration combined with foot sun bladder meridian acupoint massage can effectively promote the sputum discharge and improve the ventilation and blood gas indicators.Its clinical efficacy was better than that of the simple mechanical-assisted expectoration.
