七叶皂甙钠联合托拉塞米治疗脑出血的机制研究 被引量:11

摘要 目的:探究七叶皂甙钠联合托拉塞米治疗脑出血的机制,为脑出血患者的临床治疗提供相关帮助。方法:选取我院收治的86例脑出血患者,按随机数字表法分为对照组和联合组,各43例,对照组采用常规治疗,联合组在常规治疗基础上给予七叶皂甙钠与托拉塞米的联合治疗,治疗前后检测各组氧化应激指标谷胱甘肽过氧化物酶(GSH-Px)、过氧化氢酶(CAT)、丙二醛(MDA)、超氧化物歧化酶(SOD),血清炎症因子肿瘤坏死因子-α(TNF-α)、白介素(IL)-6、C反应蛋白(CRP)、IL-2,血液流变学指标全血黏度高切(WHV)、血浆黏度(PV)、红细胞压积(PCV)、红细胞聚集指数(PAd T)情况。结果:治疗前两组脑出血患者氧化应激指标、血清炎症因子以及血液流变学指标比较差异无统计学差异(P>0.05)。与治疗前相比,两组脑出血患者经过相关治疗后血清炎症因子(TNF-α、IL-6以及CRP)、MDA、血液流变学指标(WHV、PV、PCV以及PAd T)显著降低,氧化应激指标(GSH-Px、CAT以及SOD)以及IL-2显著升高,差异存在统计学意义(P<0.05)。与治疗后对照组相比,联合组经过联合治疗后血清炎症因子(TNF-α、IL-6以及CRP)、MDA、血液流变学指标(WHV、PV、PCV以及PAd T)显著降低,氧化应激指标(GSH-Px、CAT以及SOD)以及IL-2显著升高,组间差异存在统计学意义(P<0.05)。结论:七叶皂甙钠与托拉塞米的联合治疗降低了脑出血患者的氧化应激反应和炎症反应,有助于改善循环内血液瘀滞的状态,增加微循环的循环速度,对脑出血患者的临床治疗具有十分重要的意义。 Objective: To observe the mechanism of Sodium aescinate combined with Torasemide for cerebral hemorrhage treatment and offer clinical help to cerebral hemorrhage treatment. Methods: A total of 86 patients with cerebral hemorrhage were selected and randomly divided into groups: the observation group( 43 people) and the control group( 43 people). The patient in the control group were treated with conventional therapy and the patients in the observation group were treated with Sodium aescinate combined with Torasemide on the basis of conventional therapy. Oxidative stress indexes( GSH-Px,CAT,SOD and MDA),serum inflammatory factors( TNF-α,IL-6,CRP and IL-2) and hemorheology indexes( WHV,PV,PCV and PAd T) were detected and analyzed before and after treatment. Results: The comparison of. Oxidative stress indexes,serum inflammatory factors and hemorheology indexes in the two groups before treatment were not statistically significant( P〉0. 05). serum inflammatory factors( TNF-α,IL-6,CRP),MDA and hemorheology indexes( WHV,PV,PCV and PAdT) in both groups after treatment significantly decreased compared with that before treatment.; Oxidative stress index( GSH-Px,CAT,SOD and MDA) and IL-2 in both groups after treatment significantly increased compared with that before treatment. The changes were statistically significant( P〈0. 05). serum inflammatory factors( TNF-α,IL-6,CRP),MDA and hemorheology indexes( WHV,PV,PCV and PAdT) in observation group after treatment decreased more significantly than that in control group,and oxidative stress index( GSH-Px,CAT,SOD and MDA) and IL-2 increased more significantly than that in control group The difference between two groups was considered statistically significant( P〈0. 05). Conclusions: Sodium aescinate combined with Torasemide could decrease the oxidative stress and inflammatory reaction in patients with cerebral hemorrhag,which is beneficial to improving the condition of circulating blood stasis,accelerating
