摘要 目的探讨螺旋CT在胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎鉴别诊断中的应用价值。方法收取2012年8月至2015年8月咸阳市第一人民医院收治的胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎患者共99例作为研究对象,对其CT诊断结果进行分析比较。结果胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎在形状、密度、平扫密度以及三期增强扫描密度方面具有一定的差异。此外,自身免疫性胰腺炎组患者主要以胰腺肿胀为影像学表现,CT结果大多为弥漫性强化减低,胰管改变以不规则狭窄为主,易累及肾脏;胰腺癌组患者主要CT表现为弥漫性密度减低,胰管改变以截断为主;胰腺内分泌肿瘤更易出现胰周包膜样边缘、血管受侵犯以及假囊肿形成。上述特征性表现与其他疾病比较均具有统计学差异(P<0.05)。结论螺旋CT在胰腺癌、胰腺内分泌肿瘤及自身免疫性胰腺炎的鉴别诊断中具有重要临床价值,可作为相关疾病定位及定性诊断的重要方法。 Objective To explore the applications of spiral CT in differential diagnosis of pancreatic cancer, pancreatic endocrine tumors(PETs) and autoimmune pancreatitis(AIP). Methods Ninety-nine patients with pancreatic cancer, PETs or AIP accepted in our hospital from Aug. 2012 to Aug. 2015 were selected into this study. CT results of patients with different diseases were analyzed and compared. Results Pancreatic cancer, PETs, and AIP had a certain difference in shape, density, plain scan density and the three phase enhanced scan density. In patients with autoimmune pancreatitis, pancreatic edema was mainly imaging findings,and was mostly diffuse enhancement. The change of pancreatic duct was mainly irregular stenosis that was easy to involve in the kidney. In patients with pancreatic cancer, the main CT showed diffuse density reduction with pancreatic duct truncation. CT results of PETs showed envelope shape edge around pancreas, infiltrated blood vessel and pseudocyst. The characteristic presentation of each disease had statistically difference compared with another two diseases(P〈0.05). Conclusion Spiral CT has important clinical value in differential diagnosis of pancreatic cancer, PETs and AIP.
