血清降钙素原与血液细菌培养对菌血症的诊断意义探究 被引量:3

摘要 目的:探究血清降钙素原(PCT)检测和血液细菌培养对菌血症诊断的价值。方法:选取2015年1月-2016年1月住院患者中菌血症患者60例为A组、肿瘤高热患者30例为B组和健康体检者30例为C组,对三组研究对象血液培养、血清PCT水平进行回顾性分析。结果:A组PCT阳性率明显高于B组及C组(P<0.05)。当菌血症高热时,大多数患者血液PCT水平已升高,但血液细菌培养阳性报告时间较血清PCT升高迟;发热后72 h血液细菌培养报阳率较发热后24、48 h明显降低(P<0.05)。感染控制组患者的血清PCT水平明显低于感染未控制组(P<0.05)。结论:连续进行血PCT检测,对菌血症病情的发展和治疗效果的预后判断具有很重要的临床意义。可疑菌血症患者接受血清PCT与血液细菌培养联合检测,可有效缩短诊断时间以及提高诊断的准确率,对抗生素的合理使用及菌血症患者的治疗和预后十分有益。 Objective:To explore the value of serum procalcitonin(PCT) detection and blood bacterial culture in the diagnosis of bacteremia.Method:Among the inpatients in our hospital from January 2015 to January 2016,a total of 60 patients with bacteremia were selected as group A,30 patients with tumor hyperthermia(group B) and 30 healthy subjects(group C) were enrolled in this study.The blood culture and serum levels of PCT in the three groups were analyzed retrospectively.Result:The PCT positive rate in group A was significantly higher than that in group B and C(P〈0.05).When the bacteremia patients were with high fever,the PCT level of blood was increased in most patients,but the time of blood bacterial culture positive was later than that of serum PCT.72 h after the fever,the blood bacteria culture positive rate significantly reduced compared with 24 h,48 h after fever(P〈0.05).The level of PCT in patients with infection control group was significantly lower than that in the not control group(P〈0.05).Conclusion:Continuous blood PCT test is good for the development of bacteremia illness and the prognostic of treatment effect.Suspected bacteremia patients with serum PCT and blood bacterial culture combined detection can effectively shorten the diagnosis time and improve the accuracy of the diagnosis,it is very useful for the rational use of antibiotics and bacteremia treatment and prognosis.
