摘要
目的探讨腹腔镜和开腹阑尾切除术治疗老年穿孔性阑尾炎的效果。方法选取我院2014年5月至2016年5月收治的老年穿孔性阑尾炎患者40例,随机分为观察组和对照组各20例。对照组采用传统开放性手术进行治疗,观察组采用腹腔镜联合手术进行治疗。比较两组患者的平均手术时间、平均术中出血量、平均住院时间、肠蠕动恢复时间和并发症发生率。结果观察组的肠蠕动恢复时间、平均住院时间和平均术中出血量均显著少于对照组,差异具有统计学意义(P<0.05);两组平均手术时间比较差异无统计学意义(P>0.05)。观察组的肺部感染、切口感染、粘连复发发生率分别为5.0%、5.0%和0.0%,显著低于对照组的30.0%、40.0%和30.0%,差异具有统计学意义(P<0.05)。结论与开腹阑尾切除术比较,采用腹腔镜阑尾切除术治疗老年穿孔性阑尾炎具有更加显著的疗效,且并发症发生率低,值得临床应用及推广。
Objective To explore the the effect of laparoscope and open appendectomy in treatment of elderly patients withperforated appendicitis. Methods 40 cases of elderly patients with perforated appendicitis admitted to our hospital from May 2014 to May2016 were randomly divided into observation group and control group, with 20 cases in each group. The control group was treated withtraditional open surgery, while the observation group was treated with laparoscopic surgery. The average operational time, averageintraoperative blood loss, average hospitalization time, recovery time of intestinal peristalsis and incidences of complications of patients werecompared between two groups. Results The recovery time of intestinal peristalsis, average hospitalization time and average intraoperativeblood loss of the observation group were all significantly less than those of the control group, with statistical difference （P 〈0.05）. Nostatistical difference was found in the average operational time between the two groups （P 〉0.05）. The incidence of pulmonary infection,incision infection and adhesion recurrence of the observation group was respectively 5.0%, 5.0% and 0.0%, significantly lower than 30.0%,40.0% and 30.0% of the control group, with statistical difference （P〈0.05）. Conclusions Compared with open appendectomy, laparoscopicappendectomy in treatment of elderly patients with perforated appendicitis has more significant effect and lower incidence of complications,which is worthy of clinical application and promotion.
