摘要
目的比较高危型人乳头状瘤病毒(high risk human papilloma virus,HR-HPV)DNA检测与液基薄层细胞学检查(thinprep cytology test,TCT)在宫颈癌早期筛查中的诊断价值。方法对2013年12月至2015年12月本院收治的1195例宫颈异常患者进行HR-HPV DNA检测和TCT,同时设计2种联合检测方法,分别为COM1(HR-HPV DNA→TCT)和COM2(TCT→HR-HPV DNA),并与组织病理活检结果对比。计算各种检测方法的诊断效能。结果 HR-HPV DNA检测的灵敏度高于TCT和COM2(P<0.05),但与COM1比较无显著差异(P>0.05)。在4种检测方法中,HR-HPV DNA检测的特异度和诊断符合率最低(P<0.05),COM1的特异度和诊断符合率最高(P<0.05)。结论 HR-HPV DNA检测与TCT在宫颈癌的筛查中各具优势。先采用HR-HPV DNA检测进行筛选,再对其中的阳性患者进行TCT,具有较高的诊断效能。
Objective To compare the diagnostic value of high risk human papilloma virus (HR-HPV) DNA and thinprep cytology test (TCT) in the diagnosis of precancerous lesion of uterine cervical carcinoma. Method 1195 cases with cervical disease in our hospital from December 2013 to December 2015 were selected and applied with HR-HPV DNA test and TCT. Two kinds of joint detection method were designed, which were COMI(HR-HPV DNA test → TCT) and COM2 (TCT →HR-HPV DNA test). Compared these testing results with the pathological biopsy results and calculated the diagnostic performance of various detection method. Result In the four kinds of detection method, HR-HPV DNA test had higher sensitivity than TCT and COM2 (P 〈 0.05). There was no significant difference of sensitivity between HR-HPV DNA test and COM1 (P 〉 0.05). HR-HPV DNA test had the lowest specificity and diagnose accordance rate (P 〈 0.05). COM1 had the highest specificity and diagnose accordance rate (P 〈 0.05). Conclusion HR-HPV DNA test and TCT have different advantages in the diagnosis of early uterine cervical carcinoma. HR-HPV DNA test firstly and applied TCT further for the patients with positive result of HR-HPV DNA test has better diagnostic efficiency in the diagnosis of early cervical cancer.
