摘要
目的探讨肝脏神经内分泌肿瘤(liver neuroendocrine tumor,LNET)的磁共振成像(magnetic resonance imaging,MRI)特征。方法回顾性分析19例LNET患者的临床病理特征及MRI特点。结果 LNET主要来源于消化系统,以胰腺最多见;其组织病理分型以G3型(63.16%)为主,G2型占26.32%,G1型占10.53%。MRI表现主要分为4类:Ⅰ型:动脉期轻度强化或环形强化,门脉期造影剂减退,延迟期呈稍低信号影并可见假包膜环形强化;Ⅱ型:动脉期轻度环形强化或周边斑片状不均匀强化,门脉期及延迟期强化范围增大,呈稍高信号;Ⅲ型为混合类型即同时存在Ⅰ型和Ⅱ型的特点;Ⅳ型为特殊类型:病变为囊实性病变。其中以Ⅰ、Ⅱ型为主。结论 MRI特点:Ⅰ型多见,Ⅱ型次之,其动脉期环形强化、延迟期假包膜形成为相对特征性表现,但诊断仍需依据病理检查。
Objective To explore the magnetic resonance imaging (MRI) characteristics of tumor markers of liver neuroendocrine tumor (LNET). Methods The clinical pathological and MRI features of 19 patients were retrospectively analyzed. Results LNET mainly came from the digestive system, the most common was pancreas. The histopathologie classifications were given priority to G3 type (63.16%) , however, G2 type occupied 26.32% , G1 type accounted for 10.53%. MRI performance was mainly divided into 4 categories: type I : mild homogeneous or peripheral annular en- hancement showed in the arterial phase, contrast agents decreased in the portal venous phase, slightly low signal in the delayed phase but psuedoeapsule was visible; type Ⅱ: mild peripheral annular or peripheral patchy inhomogeneous en- hancement showed in the arterial phase, while in the portal venous and delayed phase enhancement scope increased, slightly hyperintense signal; type Ⅲ the mixed type that had the features of type Ⅰ and type Ⅱ at the same time; type IV : a particular type showed cystic-solid lesions. The majority of type Ⅰ and Ⅱ dominated MRI expression of LNET. Conclusion MRI features: type I is the most, type Ⅱ takes the second place, peripheral enhancement in the arterial phase and psuedocapsule in the delayed phase are LNET' s relatively characteristic performances, but its final diagnosis still needs to be based on pathological examination.
