摘要
目的：探讨宫腹腔镜联合及经阴道手术治疗剖宫产术后子宫瘢痕憩室的临床疗效。方法选择180例剖宫产后子宫瘢痕憩室患者的临床资料，其中 A 组90例患者行宫腹腔镜联合手术治疗，B 组90例患者采用经阴道手术治疗，观察两组治疗效果。结果全部患者治疗后月经淋漓不尽情况均消失，经期均较治疗前缩短，两组治疗后经期比较差异未见统计学意义（P ＞0.05）；A 组手术时间、术中出血量、住院时间大于 B 组，手术费用高于 B 组，差异均有统计学意义（P ＜0.05），术后并发症发生情况比较差异未见统计学意义（P ＞0.05）。A 组术后症状缓解率为94.1％，B 组为94.6％，差异未见统计学意义（P ＞0.05）。结论宫腹腔镜联合及经阴道手术均可有效治疗子宫瘢痕憩室，显著改善患者症状，但后者创伤小，患者术后恢复快，费用低，手术器械要求低，值得临床推广。
Objective To study the clinical curative effects of laparoscopy combined with hyste-roscopy and transvaginal surgery on uterine scar diverticulum. Methods The clinical data of 180 patients with uterine cesarean scar diverticulum were selected,including 90 patients in group A with hysteroscopy and laparoscopy combined surgery treatment,90 patients in group B with trans vaginal surgery,the thera-peutic effects of the two groups were observed. Results After the treatment,meno metrorrhagia situation disappeared,the menstrual cycle was shorter than before treatment in all the patients,there was no sig-nificant difference between the two groups after treatment（P 〉 0. 05）;the operation time,intraoperative blood loss,hospitalization time of group A were greater than those of group B;Operation cost of group A was higher than that of group B,there were statistically significant differences（P 〈 0. 05）,the postopera-tive complications of the two groups had no significant difference（P 〉 0. 05）. The rate of symptom relief after operation in group A was 94. 1% ,and the remission rate in group B was 94. 6% ,the difference was not significant（P 〉 0. 05）. Conclusions Laparoscopy combined with hysteroscopy and transvaginal sur-gery can be effective on uterine scar diverticulum,which can significantly improve symptoms,but the lat-ter has small trauma,postoperative quick recovery,low cost,low requirement of surgical instruments,so it is worth of clinical promotion.
