Correlation analysis of serum fibroblast growth factor 21 with insulin resistance and islet β-cell function in patients with type 2 diabetes mellitus

摘要 目的探讨2型糖尿病患者血清FGF-21水平与胰岛素抵抗和胰岛β细胞功能之间相关性。方法选取内分泌科诊治的164例糖尿病患者为研究对象,分为2型糖尿病组110例(T2DM组)和糖耐量受损组54例(IGT组),选择同期正常糖耐量的56例健康体检者作为对照组(NGT组),测定FGF-21水平并进行比较。结果 T2DM组与NGT组比较,T2DM组的BMI、SBP、TG、LDL-C、FPG、2 h PG、FINS、FGF-21、HOMA-IR均高于NGT组,HOMA-β低于NGT组,差异均有统计学意义(P<0.05)。Spearman相关分析表明,血清FGF-21与BMI、TG、LDL-C、FPG、FINS、HOMAIR呈正相关,与HOMA-β呈负相关。结论 FGF-21可能在T2DM的发生发展中起重要作用,可作为间接性预测糖尿病发病的指标之一。 Objective To investigate the correlation of serum FGF-21 with insulin resistance,islet β-cell function in patients with type 2 diabetes mellitus. Methods 164 patients with mellitus in the Department of Endocrinology were selected as the research objects,and divided into type 2 diabetes mellitus group( T2 DM group,110 cases) and impaired glucose tolerance group( IGT group,54 cases); 56 case of healthy subjects with normal glucose tolerance were selected as the control group in the same period( NGT group). The levels of FGF-21 were determined and compared. Results Body mass index( BMI),systolic blood pressure( SBP),triglyceride( TG),low density lipoprotein cholesterol( LDL-C),FPG,2 h PG,FINS,FGF-21,insulin resistance( HOMA-IR) in T2 DM group were higher than that in NGT group,while pancreatic beta cell function( HOMA-beta) was lower than that in the NGT group,and the differences were statistically significant( P 0. 05). Spearman correlation analysis showed that serum FGF-21 was positively correlated with BMI,TG,LDL-C,FPG,FINS,HOMA-IR,and negatively correlated with HOMA-β. Conclusion FGF-21 may play an important role in the occurrence and development of patients with T2 DM,which can be used as one of indicators in indirect prediction of the incidence of diabetes mellitus.
