摘要
目的:探讨CT对创伤性和动脉瘤性蛛网膜下腔出血的鉴别诊断价值。方法:选取2008年1月-2015年4月在笔者所在医院接受治疗的蛛网膜下腔出血患者160例为研究对象,其中,44例为创伤性蛛网膜下腔出血,116例为动脉瘤性蛛网膜下腔出血;均给予CT诊断,分析诊断价值。结果:创伤性蛛网膜下腔出血CT显示外侧裂池、脑沟、纵裂池、雾带状或脑内线状高密度影,伴硬膜外血肿,脑挫伤;动脉瘤性蛛网膜下腔出血均为急性突然病,CT提示为以鞍上池为中心的脑池铸型高密度影,还有的伴颞叶海马或额叶内侧直回血肿。结论:CT在创伤性蛛网膜下腔出血和动脉瘤性蛛网膜下腔出血的鉴别诊断中,能够较好的分析出血部位及分布、血液性状及破裂血管的情况等,具有十分重要和可靠的价值。
Objective： To compare the diagnostic value of CT for the diagnosis of traumatic and subarachnoid hemorrhage.Method： From January 2008 to April 2015, 160 cases with subaraehnoid hemorrhage in our hospital were randomly selected as the object of study.Among them, 44 cases of traumatic suharachnoid hemorrhage, 116 cases of aneurysmal subarachnoid hemorrhage, all patients underwent CT differentia] diagnosis, the diagnostic value was compared.Result： Traumatic subarachnoid hemorrhage, CT showed lateral cleft pool, sulcus, longitudinal pool, fog-like band or intracerebral linear high density, with epidural hematoma, cerebral contusion.Aneurysmal subarachnoid hemorrhage, were acute sudden disease, CT tips for the saddle on the pool as the center of the brain pool high density cast shadow, as well as the temporal lobe of the hippocampus or frontal lobe medial straight back to the hematoma. Conclusion： CT in the differential diagnosis of traumatic subarachnoid hemorrhage and aneurysm spinal suharachnoid hemorrhage is better analysis of hemorrhage location and distribntion, blood traits, and the rupture of a blood vessel in the situation, has very important and reliable value.
