摘要
目的比较经皮经肝胆管造影(percutaneous transhepatic cholangiography,PTC)和内镜逆行肝胆管造影途径(endoscopic retrograde cholangiograph,ERC)在胆道恶性梗阻中放置支架的优缺点。方法回顾性分析我院166例因胆道恶性梗阻放置支架引流的患者,按其放置方法的不同分成经PTC组(包括因ERC组失败后改行PTC的8例)42例和ERC组132例,其中ERC组又包括金属支架和塑料支架,并比较各组患者的成功率、引流效果及术后并发症率。结果经ERC及PTC胆道引流的成功率分别为95.2%和100.0%。两者在胆道引流效果、术后胰腺炎、胆道出血等方面无显著性差异,ERC组胆道感染、胰腺炎的发生率高于PTC组(P<0.05)。结论经ERC及经PTC胆道引流的方法均切实可行,且各有优势,正确选择合适的方法有助于提高成功率、降低并发症发生率。
Objective To compare the advantages and disadvantages of percutaneous transhepatic cholangiography（PTC） and endoscopic retrograde cholangiography（ERC） in the placement of stents in malignant biliary obstruction.Methods A total of 166 cases of patients who were placed with stent drainage due to malignant biliary obstruction in our hospital were retrospectively analyzed. According to the different methods of placement, they were assigned into the PTC group of 42 cases（including 8 cases in the ERC group changed to PTC due to failure） and ERC group of 132 cases.ERC group included metal stent and plastic stent. The success rate, drainage effect and postoperative complication rate of each group were compared. Results The successful rates of biliary tract drainage via ERC and PTC were 95.2% and100.0% respectively. There was no significant difference between the two groups in the effect of biliary drainage, postoperative pancreatitis, and biliary tract bleeding. The incidence rate of biliary tract infection and pancreatitis was higher in ERC group than those in PTC group（P〈0.05）. Conclusion Biliary tract drainage via ERC and PTC is feasible,which have their own advantages. The correct selection of the appropriate method will help to improve the successful rate and reduce the rate of complications.
