摘要
目的探讨中医体质辨识及调护对老年综合评估的临床效应。方法选取2015年3月-2016年3月人住北京市隆福医院北苑院区养老区的老年人117例,采用随机数字表法分为干预组(58例)和对照组(59例)。干预组在常规临床、护理治疗基础上,根据中医体质辨识结果进行调护,包括中医方药及中医技术干预;对照组仅予常规临床、护理治疗。两组分别在入住时、入住第4周、入住第8周和入住第12周进行日常生活能力(ADL)评分,并统计12周内住院率。结果两组人住第4、8、12周ADL评分均高于人住时(P〈0.05)。干预组人住第8、12周的ADL评分均高于对照组(P〈0.05)。干预组的12周内住院率低于对照组(P〈0.05)。结论中医体质辨识及调护纳入老年综合评估有临床应用价值和意义;依据体质差异辨证施治可提高老年人ADL和认知功能,降低住院率。
Objective To investigate clinical effect of TCM identification and nursing applied in comprehensive geri- atric assessment. Methods 117 old people in North Yuan District Old Age Area of Beijing Longfu Hospital from March 2015 to March 2016 were selected. All patients were divided into intervention group (58 cases) and control group (59 cases) by using the random number table method. On the basis of general clinical nursing, intervention group was given nurs- ing according to constitution identifications in TCM, including TCM prescription medicine and technical intervention. While control group was given only general clinical nmsing treatment. ADL score when moving in, the forth week, the eighth week and the twelfth week between two groups were assessed. 12 weeks inpatient rate was calculated. Results ADL scores of two groups in the forth week, the eighth week and the twelfth week were higher than moving in (P 〈 0.05). ADL scores of intervention group in the eighth and the twelfth week were higher than those of control group (P 〈 0.05). 12 weeks inpatient rate in intervention group was lower than that in control group (P 〈 0.05). Conclusion It's of high clinical application value to incorporate TCM identification and nursing in comprehensive geriatric assessment. Treat the elderly according to their constitutions can help improve the ADL and cognitive function of the elderly, and lower inpatient rate.
