射血分数保留的心力衰竭患者血清MMP_2,NT-proBNP与HCY的表达变化 被引量:7

摘要 目的探讨射血分数保留的心力衰竭(HF-PEF)与血清基质金属蛋白酶2(MMP_2)、B型钠尿肽前体N端(NT-proBNP)及血清同型半胱氨酸(HCY)之间的相关性。方法通过临床病例整体抽样选取射血分数保留的心力衰竭患者72例,作为HF-PEF组,按照纽约心脏病协会(NYHA)心功能分级分为Ⅱ-Ⅳ级,同期从体检中心体检者中选取心功能正常者作为对照组,26例,两组均采用酶联免疫吸附法(ELISA)测定MMP_2,同时测定血清中NT-proBNP及HCY水平,并对HF-PEF组与对照组之间三项指标进行分析比较。结果1.与心功能正常者对照组相比,HF-PEF组中的MMP_2,NT-proBNP与HCY水平均有显著性升高(P<0.05),且不同NYHA心功能分组中,三者水平也存在显著性差异(P<0.05);2.HF-PEF组患者血清中MMP_2与NT-proBNP、HCY呈明显正相关,相关系数分别为0.842、0.650(P<0.01),NT-proBNP与HCY之间呈明显正相关(r=0.592,P<0.01)。结论 HF-PEF患者血清中MMP_2,NT-proBNP与HCY水平与心功能分级与疾病的发展呈正相关,联合检测三者对HF-PEF患者的诊断和心衰程度的评估具有指导意义。 Objective To study the relationship between the heart failure with preserved ejection fraction and serum Matrix Metalloproteinase 2 (MMP_2),N-terminal Pro-Brain Natriuretic Peptide (NT-proBNP)and homocysteine (HCY).Methods The patients (n=72)with HF-PEF were chosen as HF-PEF group by using cluster sampling meth-od,and then HF-PEF group was divided grade Ⅱ-Ⅳ by NYHA.Meanwhile the persons having physical examination with normal heart function were chosen as control group (n=26).MMP_2 were determined by ELISA,and NT-proB-NP,HCY were determined simultaneously,the results were analyzed and compared between the HF-PEF group and control group.Results 1.Compared with the control group,the levels of MMP_2,NT-proBNP and HCY rised signifi-cantly in HF-PEF group (P〈0.05),and there were significant difference among grade Ⅱ-Ⅳ groups by NYHA with these three items.2.MMP_2 was positively correlated to NT-proBNP and HCY (r=0.842,r=0.650,P〈0.01),NT-proBNP and HCY were positively correlated to each other(r=0.592,P〈0.01).Conclusion The level of serum MMP_2,NT-proBNP and HCY were positively correlated to cardiac functional grading and disease development,The com-bined detection of these three items has guiding significance in the diagnosis and severity of HF-PEF.
