摘要
目的探讨麻醉处理对肿瘤患者热化疗的影响。方法选取46例2014年7月-2016年9月在本院进行热化疗治疗的肿瘤患者,选择肌注鲁米那钠、东莨菪碱进行治疗前麻醉,再静脉推注咪唑安定、芬太尼、维库溴铵、异丙酚诱导气管插管,连接麻醉机,采用异氟醚、异丙酚、维库溴铵维持麻醉。然后进行热化疗治疗,及时补液,观察并记录各时间段的各项生理指标变化情况。结果与麻醉前相比,升温后至恒温完成再降至39℃过程中患者心率、动脉血氧分压均有明显增高(P〈0.05),平均动脉压、血糖含量均有明显下降(P〈0.05),尤其恒温期时,下降明显。热化疗过程中,患者无严重并发症发生。结论对肿瘤术后热化疗患者进行麻醉处理有利于保护患者循环的稳定,并且对治疗有一定的辅助作用。
Objectives To investigate the effects of anesthesia in the treatment of cancer patients with chemotherapy. Methods A total of 46 patients, who received hyperthermia chemotherapy in our hospital between April 2014 and Septem- ber 2016, were included in the study. Luminal sodium and scopolamine were intramuscularly injected for anesthesia treatment prior to operation, then midazolam, fentany] and veeuronium bromide were intravenously injected, and then propofolinduced endotraeheal intubation was performed and connected to the anesthesia machine. Anesthesia was maintained with isoflurane, propofol and vecuronium bromide. Thereafter, the patients were treated with hyperthermia chemotherapy and immediately fluid infusion was given. The changes in various physiological indexes at different time periods were observed and recorded. Results Compared with before anesthesia, the heart rate and arterial oxygen partial pressure of patients increased significantly during the rising period of the patients temperature to the constant temperature (P 〈 0.05), while the mean arterial pressure and blood glucose levels decreased (P 〈 0.05). Especially during the period of constant temperature, mean arterial pressure and blood glucose levels decreased significantly. During the course of chemotherapy, no serious complication occurred. Conclusions Anesthesia treatment works effectively in protecting physiological functions and various organs of patients who were getting hyperthermia chemotherapy after tumor operation. It is helpful for the treatment to some extent.
