摘要
目的研究九味肝泰胶囊联合恩替卡韦对慢性乙型肝炎早期肝硬化患者血清肝纤维化血清学指标及肝功能的影响,为临床用药提供理论依据。方法将312例慢性乙型肝炎早期肝硬化患者,按随机原则分为九味肝泰组、恩替卡韦组和联合治疗组;九味肝泰组服用九味肝泰胶囊;恩替卡韦组服用恩替卡韦片;联合治疗组为前2组联合用药,治疗6个月后比较患者血清超氧化物歧化酶(SOD)、丙二醛(MDA)、转化生长因子-βl(TGF-βl)、还原型谷胱甘肽(GSH)、HBV DNA定量、血清肝纤维化血清学指标及肝功能等指标。比较总有效率以确定不同治疗方案的临床疗效。结果治疗6个月,联合治疗组血清SOD和GSH明显提高,MDA明显降低,肝功能指标谷丙转氨酶(ALT)、总胆红素(TBIL)、胆汁酸(TBA)、谷草转氨酶(AST)和谷氨酰转移酶(GGT)明显降低,与恩替卡韦组比较,P<0.05。HBV DNA定量和TGF-βl与九味肝泰组比较,P<0.05。血清肝纤维化血清学指标透明质酸(HA)、Ⅲ型胶原(PC-Ⅲ)、血清层粘连蛋白(LN)和IV型胶原(c IV)明显降低,总有效率96.11%,与九味肝泰组和恩替卡韦组比较,P<0.05。结论九味肝泰与恩替卡韦联合用药在治疗慢性乙型肝炎早期肝硬化时可产生药物协同作用,对降低HBV DNA定量、减轻肝组织纤维化病理进程、促进肝功能恢复方面较单独用药疗效更好。
Objective To study the nine kinds of Gantai combined with entecavir on serum indexes of serum hepatic fibrosis and liver function in patients with chronic hepatitis B cirrhosis of the effect of early,to provide theoretical basis for clinical therapy. Methods 312 cases of chronic hepatitis B cirrhosis patients were randomly divided into nine kinds of Gantai group and entecavir group and combined treatment group;nine kinds of Gantai group took nine kinds of Gantai Capsule;entecavir group taking entecavir tablets;combined therapy group the first two groups combined medication,6 months after treatment the serum superoxide dismutase（ SOD）,malondialdehyde（ MDA）,transforming growth factor beta-βl（TGF-βl）,glutathione（GSH）,HBV DNA,quantitative serum serological markers of liver fibrosis and liverfunction index. Comparison of the total effective rate in order to determine the clinical efficacy of different treatment options. Results After 6 months of treatment,combined treatment group serum SOD and GSH increased,MDA decreased significantly,liver function index of alanine aminotransferase（ ALT）,total bilirubin（ TBIL）,bile acid（ TBA） and aspartate aminotransferase（AST） and glutamyl transferase（GGT） decreased significantly,and entecavir group,P 〈0. 05.DNA TGF- quantitative and HBV beta L and nine flavor liver Thai group comparison,P 〈0. 05. Serological indexes of serum fibrosis hyaluronic acid（HA）,collagen type III（PC-III）,serum laminin（LN） and type IV collagen（cI V） significantly decreased,the total efficiency of 96. 11%,and nine kinds of Gantai group and entecavir group,P 〈0. 05. Conclusion Nine kinds of zhigantan and entecavir combination therapy in the treatment of chronic hepatitis B early may have synergistic effect on reducing the HBV of cirrhosis,DNA,quantitative pathological process,reduce fibrosis of liver tissue and promote recovery of liver function has better curative effect than single drug.
