摘要
目的探讨并总结鼻唇沟皮瓣的类型及采用鼻唇沟皮瓣修复鼻部皮肤缺损创面的方法。方法选取2008年10月~2014年9月本院收治的334例患者,依据鼻亚单位美学原则,鼻部创面皮肤缺损的大小、深度及部位等,选取相应鼻唇沟皮瓣大小为1.0 cm×1.0 cm^3.5 cm×2.5 cm修复缺损创面0.8 cm×0.8 cm^3.0 cm×2.0 cm。其中18例患者采用易位皮瓣、86例患者为"风筝"皮瓣、92例患者为改良菱形皮瓣、22例患者为旋转皮瓣、116例患者为皮下蒂岛状鼻唇沟皮瓣。鼻唇沟皮瓣蒂在下方患者为116例,蒂在上方患者为218例,全部创面与皮瓣匹配良好。结果本研究中创面1期愈合为330例,有4例患者部分表皮坏死,换药后愈合。术后随访230例患者,时间为1~5年,切口瘢痕不明显,视觉效果良好,患者面部形态及美学效果满意,且无一例患者肿瘤出现复发。结论患者手术依据鼻亚单位美学原则,修复鼻部肿瘤切除创面,手术操作简便,无明显改变面部形态,术后成活率高,修复后创面皮肤轮廓、质地及色泽与周围皮肤匹配较好,患者面部形态及美学效果满意,临床应用价值大,值得推广。
Objective To investigate and summarize the types of nasolabial flap,and a method of using nasolabial flap in repair of nasal skin defect.Methods 334 cases from October 2008 to September 2014 in our hospital patients were reviewed.According to the nasal aesthetic subunit principle,nasal wound skin defect size,depth and location,select the appropriate nasolabial flap,which the size is 1.0 cm×1.0 cm^3.5 cm×2.5 cm,to repair the defects of 0.8 cm×0.8 cm^3.0 cm×2.0 cm.Among them,18 cases of patients with transposition of skin flap,86 patients whith the kite flap,92 patients with modified rhomboid flap,22 patients of rotation flap,116 patients with subcutaneous pedicle island nasal nasolabial flap of nasolabial flap.The pedicles were located at the bottom for 116 cases of patients,while above patients 218 cases.All the wounds and skin flaps were good.ResultsThe wound of 330 cases were good,while 4 cases of patients with partial necrosis.230 patients were followed-up postoperatively for 1~5 years.The incision scar is not obvious with good visual effect,facial morphology and aesthetic effect of the satisfaction of patients.No patients recurred with tumor.Conclusion the nasolabial flaps is worthy of promotion with the benefits of the nasal aesthetic subunit principle,repair of nasal tumor excision wound,simple operation,no obvious the change of facial morphology,high survival rate after operation,wound repair after skin contour,texture and color with the surrounding skin,good facial morphology and aesthetic effect of patient satisfaction.
