摘要
目的观察健脾消脂方干预2型糖尿病(type 2diabetes,T2DM)大血管病变的临床疗效。方法选取T2DM大血管病变患者60例,随机分为对照组(予以阿托伐他汀钙治疗)和治疗组(予以健脾消脂方,并予以饮食及运动干预),观察治疗前后两组患者临床疗效及血清基质金属蛋白酶-9(matrix metalloprotein-9,MMP-9)、白细胞介素-1β(interleukin-1beta,IL-1β)水平、三酰甘油(triglyceride,TG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein cholestero,LDL-C)及颈动脉内膜中层厚度(carotid intima-media thickness,cIMT)的变化。结果治疗组临床疗效明显优于对照组(P<0.05)。与治疗前比较,治疗后两组患者MMP-9、IL-1β、LDL-C、TG、TC水平均显著下降(P<0.05),cIMT无明显变化(P>0.05);两组在降低MMP-9水平方面比较,差异无统计学意义(P>0.05);治疗组在降低IL-1β、TC、TG、LDL-C水平方面与对照组比较,差异均有统计学意义(P<0.05)。结论健脾消脂方能降低T2DM大血管病变患者血清MMP-9、IL-1β水平,可能具有延缓动脉粥样硬化的作用。
Objective To investigate the clinical effect of Jianpi Xiaozhi Prescription in the treatment of patients with type 2 diabetes mellitus （T2DM） complicated by macroangiopathy. Methods A total of 60 T2DM patients with macroangiopathy were enrolled and randomly divided into control group （treated with atorvastatin calcium） and treatment group （treated with Jianpi Xiaozhi Prescription and diet and exercise interventions）. The clinical outcome and changes in the serum levels of matrix metalloproteinase-9 （MMP- 9）, interleukin-ll3 （IL-I~）, triglyceride （TG）, total cholesterol （TC）, and low-density lipoprotein choles- terol （LDL-C） and carotid intima-media thickness （cIMT） after treatment was observed in both groups. Results The treatment group had a significantly better clinical outcome than the control group （P〈 0. 05）. Both groups had significant reductions in the levels of MMP-9, IL-1β, LDL-C, TG, and TC after treatment （P〈0. 05）, and there were no significant changes in cIMT （P〈0. 05）. There was no significant difference in the reduction in MMP-9 between the two groups （P）0.05）, while there were significant differences in the reductions in IL-1β, TC, TG, and LDL-C between the two groups （P〈0. 05）. Conclusion Jianpi Xiaozhi Prescription can reduce the serum levels of MMP-9 and IL-1βand delay athero- sclerosis in T2DM patients with macroangiopathy.
