新诊断2型糖尿病患者空腹血浆CTRP9水平变化与胰岛素抵抗的关系 被引量:7

摘要 目的:探讨新诊断2型糖尿病(T2DM)患者空腹血浆CTRP9水平变化及其与胰岛素抵抗(IR)的关系。方法:选取2015-01~2015-10在佳木斯大学附属第一医院内分泌科就诊的初诊T2DM肥胖患者30例为T2DM肥胖组(BMI≥28kg/m2),及初诊T2DM非肥胖患者30例为T2DM非肥胖组(BMI〈24kg/m2)。以同期接受体检的健康者30例为对照组。收集各组一般临床资料,检测空腹血糖(FPG)、空腹胰岛素(FINS)用以计算胰岛素抵抗指数(HOMA-IR),糖化血红蛋白(Hb A1C)、血脂指标、血清CTRP9和IL-6水平。T2DM肥胖组、T2DM非肥胖组、对照组之间比较采用方差分析,对CTRP9及其他因素进行单因素相关分析,和多元逐步回归分析。结果:1正常对照组CTRP9水平高于T2DM非肥胖组,T2DM非肥胖组CTRP9水平高于T2DM非肥胖组,而正常对照组、T2DM非肥胖组、T2DM肥胖组IL-6水平明显升高(P〈0.05)。2单因素相关分析显示血浆CTRP9与BMI、FPG、Hb A1C、TG、FINS、HOMA-IR及IL-6负相关,与HDL-C正相关(P〈0.01)。3以CTRP9为因变量行多元逐步回归分析,CTRP9水平与BMI、TG、HOMA-IR独立相关(P〈0.05)。结论:T2DM患者血清CTRP9水平降低可能参与了肥胖、糖脂代谢紊乱、炎症、胰岛素抵抗及糖尿病的发生、发展。 Objective:To explore the effect of C1 q/TNF- related protein 9 and insulin resistance (IR) in patients with newly diagnosed type 2 diabetes mellitus (T2DM). Methods: In the department of Endocrinology, the First Affiliated Hospital of Jiamusi University from January to October 2015, 30 obese patients newly diagnosed as T2DM (obese group, BMI≥28 kg/m2) ,30 non- obese patients newly diagnosed as T2DM (non- obese group, BMI 〈 24kg/m2), 30 healthy subjects (control group) were enrolled in this study. The general data was collected, and the levels of fasting blood glucose (FPG)FINS, insulin resistance index(HOMA- IR), HbA1C, TC, TG, LDL -C, HDL- C, CTRP9 ,IL- 6 were calculated. Variance analysis was used to compare means. The correlation between CTRP9 and other factors were analyzed. Results: (1)The levels of CTRP9 reduced in control groups, obese, non - obese (P 〈 0. 05 ), but the level of IL - 6 reduced (P 〈 0. 05 ). (2)Single factor correlation analysis showed that the plasma level of CTRP9 had negative correlation with BMI,FPG,HbA1C,TG,FINS,HOMA -IR and IL - 6, and positive correlation with HDL - c (P 〈 0.01 ). (3)Multiple linear regression showed that BMI, TG and HOMA- IR were independently associated with plasma CTRP9 level (P 〈 0.05). Conclusion: The reduce of CTRP9 levels may be involved in obesity, glycolipid disorder, inflammation and the occurrence and development of IR and T2DM.
