摘要
目的:探讨全科团队服务模式在糖尿病患者监测随访细节管理中的应用效果。方法:选取2014年8月—2015年2月240例糖尿病患者为研究对象,随机分为对照组和干预组每组120例,对两组患者干预前后的糖尿病知识掌握情况、饮食行为、治疗依从性、自我监测行为及空腹血糖(FBG)、餐后2 h血糖(2h PG)、糖化血红蛋白(Hb A1c)、收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)水平进行比较。结果:干预组的糖尿病知识掌握情况良好率高于对照组,饮食行为及治疗依从性和自我监测行为较佳率也均高于对照组,FBG、2h PG、Hb A1c、SBP、DBP、TC水平均低于对照组,差异有统计学意义(P<0.05)。同时,干预组的规范化管理率也高于对照组。结论:全科团队服务模式在糖尿病患者监测随访细节管理中的应用效果较佳,可全面提升患者的疾病知识掌握情况和相关行为,有效改善血糖等代谢指标。
Objective： To explore the application effect of the general practitioner team service mode in the management of monitoring follow-up details of the patients with diabetes. Methods： From August 2014 to Feb. 2015, 240 cases of diabetes were selected as the research subjects and randomly divided into a control group and an intervention group with 120 cases each, and the diabetes knowledge, dietary behaviors, compliance with treatment, self-monitoring behavior, fasting blood glucose（FBG）, postprandial 2h blood glucose（2hPG）, glycated hemoglobin（HbAlc）, systolic blood pressure（SBP）, diastolic blood pressure（DBP）, and total cholesterol（TC） level were compared before and after the intervention. Results： The good rate of diabetes knowledge of the intervention group was higher than that of the control group, the good rates of dietary behaviors, treatment compliance and self-monitoring behavior were higher in the intervention group than those in the control group, and the levels of FBG, 2hPG, HbAlc, SBP, DBP, and TC of the intervention group were lower than those of the control group, and the differences had the statistical significance（P〈0.05）. At the same time, the standardized management rate of the intervention group was also higher than that of the control group. Conclusion： Application of the general practitioner team service mode in the management of monitoring follow-up details of diabetes patients is good and can enhance the mastery of disease knowledge and related behavior of the patients comprehensively and effectively improve blood glucose and other metabolic indicators.
