摘要
目的了解膀胱肿瘤电切术后患者1年内的各相关因素对于依从性的影响并分析其原因,为患者提供个性化的、高目的性的、强计划性的护理和治疗。方法调查方法 :(1)表格法;(2)谈话法;(3)观察法,综合分析。结果膀胱肿瘤患者术后1年内各阶段均存在依从性问题,而且随着时间的推移,患者依从性问题逐渐更为突出;引起患者依从性问题的原因在各阶段不同。术后1周主要因素依次为,(1)拔出尿管对正常排尿的影响;(2)首次灌药的心理反应;(3)药物的不良反应。术后3个月主要因素依次为每周1次的膀胱灌注、药物不良反应、工作问题、健康心理控制源;术后1年主要因素依次为健康心理控制源、药物不良反应、工作问题、家庭社会支持、麻痹、疏忽问题。结论膀胱肿瘤电切术后各阶段均存在不同的依从性问题,针对影响依从性问题的因素和形成原因进行分析。以此为依据为患者提供个性化的、高目的性的、强计划性的护理及辅导治疗,使用一切可能的途径来提高患者的依从性,减少复发,从而延长患者生命、提高生活质量。
Objective To learn all relevant factors of transurethral bladder tumor patients within one year for the compliance of and analyze the reasons,to provide patients with personalization,high purpose,and strong plan of care and treatment. Methods（1）tabular method,（2）talking method,（3） observation and analysis. Results There was a difference in the compliance of patients with bladder cancer at different stages after operation in one year,as time goes on,the difference was significantly. The reasons of difference is different in each phase. Main factors in postoperative one week.（1）the influence of pulling out the urine tube on normal urination.（2）psychological reaction of the first time irrigation of bladder.（3）the drug adverse reactions. Main factors in postoperative three months： the irrigation of bladder weekly,the drug adverse reactions,the work issues,the locus of mental health control. Main factors in postoperative one year： the locus of mental health control,the drug adverse reactions,the work issues,the support of family and social,the Negligence. Conclusion There was a difference in the compliance of patients with bladder cancer at different stages,analyzing the factors that affect compliance and reasons. As a basis to provide personalized,high purpose and planned care for patients,using all possible ways to improve patient compliance and reduce recurrence,thus prolonging the lives of patients,improving the quality of life.
