胃癌FOLFOX4方案化疗患者预后与血清可溶性E-钙黏蛋白表达的关系 被引量:9

Relationship between serum soluble E - cadherin expression and prognosis in gastric cancer patients treated by FOLFOX4 regimen

摘要 目的探讨血清可溶性E-钙黏蛋白(sEC)表达与胃癌患者行FOLFOXd方案化疗疗效及预后生存的关系。方法42例患者采用FOLFOX4方案化疗3个周期,观察化疗疗效,进行生存随访。采用RNA干扰技术抑制胃癌SGC-7901细胞中E-cadherin表达。酶联免疫吸附试验(ELISA)测定胃癌患者化疗前、化疗第8周血清及细胞培养液上清中sEC表达水平。比较分析化疗前后血清sEC表达变化与临床病理参数、近期疗效、无进展生存时间(PFS)和生存时间(OS)的关系。计数资料采用χ2检验,计量资料采用t检验,采用Kaplan-Meier法进行生存分析。结果化疗前与化疗后第8周比较,血清sEC表达水平差异无统计学意义[(2.269±0.655)mg/ml比(2.231±0.638)mg/ml]。化疗前后sEC表达变化与肿瘤分期、PFS和OS存在显著相关。有效组化疗后血清sEC表达水平较化疗前显著下降[(2.617±0.711)mg/ml比(1.949±0.458)mg/ml,P〈0.01,t=5.790],进展组表现为化疗后血清sEC表达水平较化疗前升高[(1.963±0.408)mg/ml比(2.612±0.779)mg/ml,P〈0.01,t=-3.665]。胃癌患者化疗前血清sEC高表达水平与近期疗效、PFS和OS显著相关。E-cadherin表达干扰显著抑制了培养液上清中sEC的表达,5-氟尿嘧啶(5-Fu)可诱导其表达增加。结论胃癌患者血清sEC可能是FOLFOX4方案化疗疗效及预后生存的独立预测因子。 Objective To investigate the association of serum soluble E - cadherin (sEC) expres- sion before and after chemotherapy with the effects and prognosis in gastric cancer patients treated by FOLFOX4 regimen. Methods Chemotherapy was performed on 42 patients with gastric cancer using three cycles of FOLFOX4 regimen, tumor response adverse effects were observed, and survival was also followed up. E - cadherin was downregulated by small RNA interference in gastric cancer SGC - 7901 cells. Enzyme linked immunosorbent assay (ELISA) was used to determine the sEC expression levels in cell cul- ture supernatant and serum of gastric cancer patients before chemotherapy and at 8th week. Comparative a- nalysis was performed for the serum expression of sEC before and after chemotherapy with clinical pathologi- cal features, short - term effects, progression free survival (PFS) and overall survival (OS) respectively. Statistical analyses were done using the SPSS 17. 0 package, with χ2 test for count data, t test for measure- ment data, and Kaplan - Meier method for survival analysis. All tests of statistical significance were two - sided and P 〈 0.05 was considered as statistically significant difference. Results Mean serum levels of sEC expression did not show significant difference before chemotherapy compared to those at 8th week after chemotherapy [ (2. 269 ±0. 655) vs. (2. 231 ±0. 638) mg/ml]. Changes in sEC levels before and after chemotherapy in patients were significantly correlated with tumor stage, short - term effect, PFS and OS. Serum sEC expression levels in effective groups decreased significantly after chemotherapy as compared withthose before chemotherapy [ (2. 617 ± 0. 711 ) vs. ( 1. 949 ± 0. 458 ) mg,/ml, P 〈 0. 01, t = 5. 790 ], and those increased significantly in progressive group after chemotherapy [ ( 1. 963 ± 0. 408 ) vs. ( 2. 612 ± 0. 779) mg/ml,P 〈 0. 01, t = - 3. 665 ).
