摘要
目的探讨生育期妇女采用不同子宫术式治疗的临床疗效,并分析影响卵巢功能的因素。方法回顾性分析2012年3月至2015年9月因生殖系统疾病而择期行手术治疗的135例生育期妇女的临床资料,分别于患者术前及术后6个月检测其卵泡刺激素(FSH)、血清雌二醇(E2)、血清抗勒氏管激素(AMH),观察其变化情况等。结果患者治疗后无更年期现象发生,且术前及术后6个月FSH、E2水平比较差异未见统计学意义(P〉0.05)。年龄超过40岁的患者术后AMH水平明显低于术前(P〈0.05),子宫切除术后AMH水平明显低于手术前(P〈0.05),子宫次全切除术后AMH水平与术前比较差异未见统计学意义(P〉0.05),子宫全切除术与广泛性子宫切除术后AMH水平明显低于手术前(P〈0.05),行腹腔镜下子宫切除术治疗者术后AMH水平低于手术前(P〈0.05)。结论生育期妇女采用不同子宫术式治疗生殖系统疾病在短期内不会使卵巢功能衰竭,而影响术后卵巢储备功能下降可能与子宫切除方式、年龄、途径等存在密切关系。
Objective To investigate the clinical efficacy of different surgical treatment of uter-ine in women of reproductive age, and analyze the influencing factors of ovarian function. Methods The clinical data of 135 women of reproductive age with reproductive system diseases received operation from March 2012 to September 2015 were retrospectively analyzed. The changes of the follicle stimulating hor-mone (FSH) , estradiol ( E2 ) , anti-mullerian hormone serum (AMH) preoperative and postoperative six months were observed. Results No menopause occur after treatment, and FSH, E2 level had no signifi- cant difference before and after six months (P 〉 0. 05 ). AMH levels after operation were significantly low- er than before surgery in patients older than 40 years old (P 〈 0. 05 ), AMH levels after hysterectomy were significantly lower than before surgery( P 〈 0. 05 ), AMH levels after subtotal hysterectomy had no signifi- cant difference compared with before surgery ( P 〉 O. 05 ), AMH levels after total hysterectomy and exten- sive hysterectomy were significantly lower than before surgery (P 〈 0.05 ), AMH levels after laparoscopic hysterectomy were lower than before surgery (P 〈 0. 05 ). Conclusions Women of reproductive age with different uterine surgical treatment of reproductive system disorders does not make ovarian failure in the short term, and the impact of postoperative diminished ovarian reserve may have close correlation with hysterectomy details, age, and other ways.
