摘要
目的探讨肺结核并发下呼吸道感染的易感因素。方法选取成都市公共卫生临床医疗中心2014年8月1日至2015年7月31日确诊的肺结核并发下呼吸道感染的住院患者作为并发症组（198例）；选取同期确诊的肺结核住院患者作为对照组（216例）。收集研究对象一般人口学特征、生活史、现病史、并发症及免疫状况等信息，分析肺结核并发下呼吸道感染的易感因素。结果结核性支气管扩张、毁损肺、低氧血症或呼吸衰竭、糖尿病、贫血、低蛋白血症、年龄≥70岁、有自身免疫病者，并发症组分别占29．29％（58／198）、15．15％（30／198）、36．36％（72／198）、19．70％（39／198）、27．27％（54／198）、27．78％（55／198）、7．07％（14／198）、21．21％（42／198），均高于对照组的12．50％（27／216）、2．31％（5／216）、6．48％（14／216）、3．70％（8／216）、18．52％（40／216）、14．81％（32／216）、1．85％（4／216）、3．70％（8／216），差异均有统计学意义（χ^2值分别为17．85、21．99、56．00、26．27、4．51、10．46、6．77、29．80，P值均〈0．05）。结论对于有并发症及免疫状况差的肺结核患者，应警惕其并发下呼吸道感染。
Objective To explore risk factors of pulmonary tuberculosis complicated with lower respiratory infection. Methods From August 1, 2014 to July 31, 2015, hospitalized patients with pulmonary tuberculosis complicated with lower respiratory infection from Chengdu public health clinical center were included as complication group （n=198）; pulmonary tuberculosis patients were selected as control group （n=216）. Their general demo- graphic characteristics, life history, current history, complications and immune status were collected to analyze the predisposing factors of pulmonary tuberculosis complicated with lower respiratory tract infection. Results Tuber- culosis bronchiectasis, lung damage, hypoxia or respiratory failure, diabetes, anemia, hypoproteinemia, age of 70 years or older, autoimmune disease and complications accounted for 29.29% （58/198）, 15.15% （30/198）, 36.36% （72/198）, 19.70% （39/198）, 27.27% （54/198）, 27.78% （55/198）, 7.07% （14/198） and 21.21% （42/198）, respectively in complication group; which were significantly higher than those of control group （12.50% （27/216）, 2.31% （5/216）, 6.48% （14/216）, 3.70% （8/216）, 18.52% （40/216）, 14.81% （32/216）, 1.85% （4/216） and 3.70% （8/216）, respectively） （χ^2=17.85, 21.99, 56.00, 26.27, 4. 51, 10.46, 6.77 and 29.80, all P values 〈 O. 05 ）. Conclusion Pulmonary tuberculosis patients with complications and poor immune status should be alert to their concurrent lower respiratory tract infections.
