摘要
目的对比三种不同治疗方案对于左半结肠癌急性肠梗阻患者的效果。方法选取本院就诊的30例左半结肠癌急性肠梗阻患者,采用三种不同的治疗方案进行比较,各组10例患者,A组采用切除吻合术联合全结肠灌洗疗法,B组采取切除吻合术治疗,C组通过切除吻合近端造瘘术手术治疗,对三种手术方式治疗左半结肠癌急性肠梗阻进行效果评价。结果三组术后都出现明显的发热症状,而腹胀、吻合口瘘、伤口并发症发生较少,A组甚至未出现吻合口瘘、伤口并发症。B组手术时间显著低于其他两组(P<0.05),A组的住院费用、手术出血量和住院时间均低于其他两组,差异具有显著性(P<0.01)。结论三种手术方式可有效地治疗左半结肠癌急性肠梗阻,此外,Ⅰ期切除吻合术联合全结肠灌洗治疗方案治疗效价最高。
Objective To compare the therapeutic effects of different surgical methods in the treatment of acute intestinal obstruction in patients with left colon cancer.Methods 30 cases of left colon cancer with acute intestinal obstruction patients according to the type of surgery will divided into A,B,C three groups,a group of 10 cases,underwent one-stage resection and anastomosis combined with total colon lavage therapy,group B（n=10）,underwent one-stage resection and anastomosis,group C（n = 10）,accept one-stage resection and anastomosis of the proximal part of the fistulation.The incidence of postoperative complications,operation time,hospitalization expenses,the amount of bleeding and length of stay of the patients in the three groups were compared.Results three groups of postoperative patients with obvious symptoms of fever,and abdominal distension,anastomotic leakage,wound complications were less,A group did not even appear anastomotic leakage,wound complications.The operation time of B group was significantly lower than that of the other two groups（P0.05）,the hospitalization expenses,the amount of bleeding and hospitalization time of A group were lower than those of the other two groups,the difference was significant（P〈0.01）.Conclusion three surgical methods can be effective in the treatment of left colon cancer with acute intestinal obstruction,in addition,I resection and anastomosis combined with the whole colon lavage treatment is the most effective method.
