摘要
目的观察复方多黏菌素B软膏联合复方酮康唑软膏治疗小腿慢性肥厚性湿疹临床疗效及安全性。方法入选84例小腿慢性肥厚性湿疹患者,随机分为两组,治疗组42例,外用复方酮康唑软膏及复方多黏菌素B软膏,一日2次(早晚各1次),连用3周;对照组42例,单用复方酮康唑软膏,一日2次(早晚各1次),连用3周,随访12周。对患者治疗前后临床症状和体征进行评分,疗效评价标准参照湿疹面积及严重程度指数(EASI)评分法。观察并记录患者复发和不良反应情况。结果治疗结束后,治疗组各临床症状和体征及皮损面积改善程度优于对照组,组间差异有统计学意义(P<0.05或P<0.01);治疗组有效率(90.48%)高于对照组(59.52%),组间差异有统计学意义(P<0.01)。治疗组复发率低于对照组(7.14%对42.90%),差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论复方多黏菌素B软膏联合复方酮康唑软膏治疗小腿慢性肥厚性湿疹临床疗效优于单用复方酮康唑软膏,并可减少复发,是一种值得临床推广的治疗方法。
Objective To investigate the efficacy of compound polymyxin B cream combined with compound ketoconazole ointment in the treatment of chronic shank hypertrophic eczema. Methods A total of 84 patients with chronic shank hypertrophic eczema were selected and randomly divided into two groups. The study group were treated with compound polymyxin B cream combined with compound ketoconazole ointment （n=42）, the control group were treated with compound ketoconazole ointment only （n=42）. The treatment was as twice a day continued for 3 weeks. The itching degree and other clinical symptoms and signs were evaluated before and after treatment in each group. The recurrence and adverse reactions were observed and recorded. Results After treatment, the clinical symptoms and signs and skin area lesions in study group were improved more effectively than those in control group （P〈0.05 or P〈0.01）. The efficacy rate in study group （90.48%） was higher than that in control group （59.52%, P〈0.01）. The recurrence rate of study group was lower than that in control group （7.14% vs 42.90%）, and the difference was statistically significant （P〈0.05）. The incidence of adverse reactions between the two groups was no significant difference（P〉0.05）. Conclusion The therapy of compound polymyxin B cream combined with compound ketoconazole ointment can significantly improve the symptoms of the patients with chronic shank hypertrophic eczema, which has a higher effective rate and lower recurrence rate compared with compound ketoconazole ointment treatment only. The combination therapy deserves further clinical application.
