摘要
目的探讨胸腔镜手术对周围肺结节的诊断与治疗价值。方法选择周围肺结节患者105例,分别采用VATS和肺穿刺活检对周围肺结节患者进行明确诊断,对比2种诊断方法的准确率。对病理诊断为肺癌的患者进行肺癌根治手术,将采用VATS的患者纳入VATS根治组,将采用肺穿刺活检患者纳入开胸手术组,对比2组患者的疗效。结果肺穿刺活检组明确诊断49例,准确率为81.7%;VATS组患者全部明确诊断,准确率为100.0%。2种方法对肺结节诊断的准确率和并发症存在统计学差异,P<0.05。VATS组患者术中出血量、下床活动时间和VAS评分明显低于开胸组患者,P<0.05;手术时间、清扫淋巴结个数和术后引流量方面2组无统计学差异,P>0.05。结论 VATS对肺周围结节的诊断准确率高,可作为首选的诊断方法。VATS对肺周围恶性结节的治疗效果与传统开胸手术相当,但是VATS创伤小、恢复快,可降低患者疼痛,是治疗肺周围恶性结节值得推荐的手术方法。
Objective To explore the value of video assisted surgery in the diagnosis and treatment of pulmonary nodules. Methods 105 cases of peripheral pulmonary nodules were selected. VATS and lung biopsy were used to make a definite di- agnosis of peripheral pulmonary nodules and compared the accuracy of the 2 diagnostic methods. The pathological confirmed lung cancer patients treated with radical surgery, VATS diagnosed patients were included in VATS radical group, the patients with lung biopsy were included in the thoracic surgery group,the efficacy of the 2 groups were compared. Results Lung biopsy group were clear diagnosed 49 cases,the accuracy rate was 81.7%, VATS group of patients were all clear diagnosed, the accuracy rate was 100.0%. There were significant differences in the accuracy and complications of the 2 groups with pulmonary nodules ,P 〈 0.05. The intraoperative bleeding,time of ambulation and VAS score were significantly lower in the VATS group than the open chest group, P 〈 0.05, operation time, number of lymph node dissection and postoperative lead flow rate were not significantly different, P 〉 0.05. Conclusion VATS diagnosis of pulmonary nodules is high,which can be used as the preferred diagnostic method. The therapeutic effect of VATS on the malignant nodules around the lung is comparable to the traditional open chest surgery, but VATS trauma is small with fast recovery, can reduce the pain of patients. It is a recommended surgical procedure for the treatment of peripheral lung cancer.
