摘要
目的:观察经皮与开放椎弓根螺钉系统配合正骨胶囊治疗中青年胸腰椎骨折的临床疗效。方法:选取2014年1月—2015年12月本院骨伤科二病区收治的中青年胸腰椎骨折患者60例,均经临床体格检查以及影像学资料确诊,随机分为观察组和对照组,每组30例。观察组患者行经皮椎弓根螺钉系统术治疗,对照组患者行开放椎弓根螺钉系统术,两组患者术后第2天均口服正骨胶囊治疗。结果:观察组术中出血量、术后首次下地时间均显著低于对照组(P<0.05),但观察组手术时间与术中透视次数均显著高于对照组(P<0.05);观察组与对照组术后第1天、术后4个月的椎体前缘高度比均显著高于术前(P<0.05),但两组间比较无显著差异(P>0.05);观察组与对照组术后1个月、术后4个月的VAS评分、ODI评分均显著低于对术后第1天(P<0.05),但观察组术后1个月的VAS评分、ODI评分显著低于对照组(P<0.05)。结论:经皮与开放椎弓根螺钉系统配合正骨胶囊治疗中青年胸腰椎骨折都能取得不错的疗效,需要根据临床具体情况进行选择。
Objective ：To observe the clinical curative effect of percutaneous pediele screw and open pedicle screw in combination with Zhenggu Capsules （ZGC） on young and middle-aged thoracolumbar fracture. Methods： Sixty patients with young and middle- aged thoracolumbar fracture who were confirmed by clinical physical examination and imaging data and treated in No. 2 Ward of Department of Orthopaedics and Traumatology in our hospital between Jan. 2014 and Dec. 2015 were chosen as the research sub- jects, and randomly divided into the observation group and the control group, with 30 cases in each group. The ones in the observa- tion group were treated with percutaneous pedicle screw,while the ones in the control group were treated with open pedicle screw. The ones in two groups were given ZGC on the 2rd day after the operation. Results：The amount of bleeding during the operation and the first time for getting out of bed after the operation in the observation group were significantly lower than those of the con- trol group （P 〈0.05） ,but its length of operation and intra-operative perspective times were significantly higher than those of the control group （ P 〈 0.05 ）. The two groups＇ vertebral anterior height ratio were significantly higher than those before the operation on the 1 th day and the 4th month after the operation （ P 〈 0.05 ）, but there were no obvious difference between the two groups （ P 〉 0.05 ）. The two groups＇ VAS score and ODI score on the 1th month and the 4th month after the operation were significantly lower than those on the first day after the operation （P 〈0.05）, but the observation group＇s VAS score and ODI score on the 1st month after the operation were significantly lower than those of the control group （ P 〈 0.05 ）. Conclusion： Percutaneous pedicle screw and open pedicle screw in combination with ZGC has a remarkable clinical curative effect on young and middle-aged thora- columbar fracture, and can be used according to exact clinical state.
