摘要
目的 评价强化局部麻醉在微波消融治疗肝癌过程中的安全性和有效性. 方法 100例原发性肝癌患者在微波治疗时采用强化局部麻醉.在术前、术中、术后对患者进行血压、心率、呼吸和血氧饱和度的检测.结果 所有患者均顺利完成手术.43%（43/100例）患者术中收缩压升高,18%（18/100例）诊断为围手术期高血压,给予乌拉地尔、哌替啶静脉输注治疗;26%（26/100例）患 者术中出现心率增快,最高达120次/分,74%（74/100例）患者术中出现心率降低,19%（19/100例）心率低于60次/分,给予静脉注射阿托品治疗;67%（67/100例）患者术中出现呼吸加快,17%（17/100例）患者术中呼吸减慢;16例患者术中呼吸频率无变化.无患者血氧饱和度〈90%或需要面罩加压辅助通气.术后患者血压、呼吸、血氧饱和度均趋于稳定.结论 血流动力学指标在术中及术后均保持容许范围内.在微波治疗肝癌过程中采用强化局部麻醉是安全的和有效的.
gObjective To evaluate the safety and efficiency of strengthening local anesthesia for patients with hepatic carcinoma undergoing microwave ablation. Methods 100 cases of patients with hepatic carcinoma undergoing microwave ablation by strengthening local anesthesia were studied. The blood pressure、heart rate、respiratory rate and oxyhemoglobin saturation were supervise before、during and after ablational operation. Result All 100 patients underwent the ablational operation successfully. Elevated systolic blood pressure were founded in 43% （43/100） cases patients during the operation.18 cases of patients were diagnosed with perioperative hypertension, and receive urapidil hydrochloride and pethidine intravenous injection treatment. Heart rate increases were founded in 26 cases patients during the operation, and 120 beats per minute quickest. Slow Rhythm of heart founded in 74 cases patients during the operation. Furthermore,less than 60 beats per minute were founded in 19 cases and atropine was administered. accelerated breathing、Breathing slows and inalterability of Breath were founded in 67、17 and 16 cases,respectively. It is not founded that the cases with oxyhemoglobin saturation less than 90% and the patient mask pressure assisted ventilation needed. Conclusion The cardiovascular system indicators of all 100 patients underwent ablation kept within the allowable range. The strengthening local anesthesia for patients with hepatic carcinoma und ergoing microwave ablation was safed and efficient
