摘要
目的探讨胎儿脐动脉与大脑中动脉阻力指标在胎儿缺氧预测中的应用价值。方法选取妊娠孕妇120例,其中正常妊娠者62例为对照组,胎儿缺氧妊娠者58例为观察组,应用彩色多普勒超声检测仪检测胎儿脐动脉、大脑中动脉的搏动指数(pulsatility index,PI)、阻力指数(resistance index,RI)以及收缩期峰值和舒张末期流速之比(peak systolic velocity/end diastolic velocity,S/D),计算比较脐动脉、大脑中动脉的各项阻力指标比值。结果观察组胎儿脐动脉阻力指标均明显高于对照组(P<0.01),大脑中动脉阻力指标均明显低于对照组(P<0.01);胎儿脐动脉、大脑中动脉各项阻力指标比值PI_(脐动脉/大脑中动脉)、RI_(脐动脉/大脑中动脉)及S/D脐动脉/大脑中动脉均高于对照组,差异有统计学意义(P<0.01),并以PI_(脐动脉/大脑中动脉)>1.00、RI_(脐动脉/大脑中动脉)>1.00和S/D脐动脉/大脑中动脉>0.72作为临界值,预测胎儿缺氧的敏感度、特异度及Youden指数均较高。结论胎儿脐动脉、大脑中动脉的各项阻力指标及其比值在胎儿缺氧预测中具有重要价值,能够预测胎儿缺氧以及其他不良的妊娠现象,应及早发现异常并指导临床干预,从而降低围产期新生儿病死率,提高新生儿质量。
Objective To discuss the application value of fetal umbilical artery and middle cerebral artery resistance index in predicting fetal hypoxia.Methods A total of 120 pregnant women in hospital was selected and divided into control group and observation group,the control group with 62 cases of normal pregnancy,the observation group with 58 cases of fetal hypoxia pregnancy,the use of color Doppler ultrasound detection of fetal umbilical artery,middle cerebral artery,pulsatility index(PI),resistance index(RI)and peak systolic and end diastolic velocity ratio(S/D),and calculate their correspongding ratio,and the two groups of data were statistically analyzed.Results The umbilical artery resistance index in the observation group was significantly higher than that in the control group(P〈0.01).The middle cerebral artery resistance index was significantly lower than that in control group(P〈0.01).The ratio of PIumbilical artery/middle cerebral artery,RIumbilical artery/middle cerebral arteryand S/Dumbilical artery/middle cerebral arteryof fetal umbilical artery and middle cerebral artery were significantly higher than those in the control group(P〈0.01),and had a good diagnosis effect on the sensitivity,specificity and Youden index by PI_(umbilical artery/middle cerebral artery)1.00,RIumbilical artery/middle cerebral artery 1.00 and S/D umbilical artery/middle cerebral artery 0.72.Conclusion Resistance indexes and their corresponding ratio of fetal umbilical artery and middle cerebral artery in predicting fetal hypoxia has important value,which was capable of predicting fetal hypoxia and other adverse pregnancy phenomenons,and can detect the early abnormalities to reduce neonatal mortality rate,guide the clinical intervention,improve the quality of newborn birth.
